On top of being an elite facility overall, this facility also performed well in the area of inspections, where it earned an A+. Few nursing homes fared as well in this category. We take several aspects of a facility's inspection report into consideration in calculating these inspection scores. One critical factor is health deficiencies. You should keep in mind that the severity of deficiencies is usually more meaningful than the quantity of deficiencies, as some are relatively insignificant. While this place had some deficiencies on its report, none of them were major deficiencies based on CMS' deficiency scale. Finally, this facility had no substantiated complaints this year from residents, which is an excellent sign.

Contributing to this nursing home's list of achievements, it also received a top-shelf long-term care grade. We gave them an elite grade of A- in this category. In a long-term care environment, the nursing home's primary objective is to maintain residents' quality of life and keep them safe. This nursing home's vaccination records weren't as favorable some of its other statistics in this area, such as its nursing hours data. This nursing home provided the pneumonia vaccine to just 81.74603 percent of its patients. We would like to see some improvement in this area next year. Lastly, we looked at the facility's number of hospitalizations. With 1.75 hospitalizations per one thousand long-term resident days, this nursing home is close to average in this area.

Additionally, this facility also received favorable short-term care scores this year. In fact, we awarded it one of our better grades in that category, with a grade of B+. In computing our short-term care scores, we analyze a nursing home's levels of highly skilled nursing, which includes registered nurses, physical therapists occupational therapists and other licensed professionals. This area is considered to be a fair assessment of the nursing home's ability to rehabilitate patients. One of this nursing home's strength is that it offers more hours of physical therapy per week to its residents. The last statistic we assessed in this area is the number of residents who ultimately returned home from the facility. We found that it fared as well as just about any nursing home in Indiana in this area with 56 percent of its patients returning home. For most facilities, fewer than half of their short-stay residents are able to return home.

The next category we analyzed was nursing. In that area, we awarded this nursing home a solid grade of C. This actually proved to be this facility's weakest category. Nursing scores are largely associated with the nursing home's level of nurse staffing. This nursing home offers extremely high levels of nursing care, averaging 4.2 hours per patient per day. This is well above average. Lastly, we also baked a few quality measures into our nursing grades. This nursing home fared poorly in two of the major metrics we looked at, with below average marks for minimizing its patients' pressure ulcers and major falls.
