Windsor Post-Acute Healthcare Center of Modesto is a large facility in Modesto, California. It looks like this facility is a relatively poor nursing home. A score of this caliber suggests we found a few red flags. We ranked this as one of the bottom five rated nursing homes in Modesto. With plenty of other facilities in the city, you should be able to do better. We wouldn't blame you if you're ready to stop reading and find another nursing home. Short-term Care Quality
Although this nursing home's overall score wasn't a highlight, it actually earned a good grade in the area of short-term care. We gave it a B for that area, which is a solid score. This one of its few redeeming features. Short-term care ratings are based on the facility's quantity of skilled nursing services. This includes a broad range of nursing services, ranging from registered nurses to physical and occupational therapists, not to mention other types of therapy. Fortunately, this nursing home offers both physical therapists and registered nurses. Not all facilities offer both. The final measure we assessed in this area is the number of patients that who were able to eventually return home from the nursing home. We found that it performed better than most nursing homes in this area with 52.3 percent of its patients returning home. Unfortunately, at most nursing homes, less than half of their short-term patients are able to return home.
Long-term Care Quality
This facility's next highest category turned out to be long-term care. In that area, we gave this nursing home a B-. In calculating our long-term care grades, we analyze the nursing home's ability to maintain its permanent residents' health and quality of life. Once we assessed the volume of care provided by nurses aids and other staff, we then analyzed the facility's vaccination record. This nursing home vaccinated 99.28187 percent of its patients against pneumonia, which is more than the average nursing home. Lastly, we looked at the facility's number of hospitalizations. Here we found that this place had 2.56 hospitalizations per one thousand long-term resident days. Unfortunately, this figure was not quite as favorable than its other scores in this category.
Nurse Quality
We awarded this facility a respectable grade in the area of nursing as well, with a grade of C. The nursing score weighs quite a few subcategories, however, the primary one is the number of nurse hours per patient per week. This nursing home provides 3.7 hours of nursing care per resident per day. Lastly, this place was relatively weak in two of the areas we look at, with below average scores for minimizing its residents' major falls and pressure ulcers.
Facility Inspections
Turning the final category of inspections, this facility really did not fare well. It received an F in our inspection category. For places with bad inspection scores, you should focus on any severe deficiencies found on their inspection reports. This facility was assessed 3 deficiencies by CMS. The only positive thing we can say is that none of its deficiencies were in the categories that indicate they created a threat to patient health or safety. Finally, this facility also received 9 substantiated complaints in recent years. This is yet another bad sign.
