Wedgewood Care Center is located in Grand Island, Nebraska. We gave it an overall grade of C, which is a middle of the road score. We gave this nursing home an A- for our short-term care grade. This is far more impressive than the facility's overall grade. Our short-term care scores are believed to be most important for patients requiring rehabilitation services. Rehabilitation generally mandates higher levels of highly-skilled nursing services. This means not just nursing, but also physical and respiratory therapy, as well as other variations of therapy. This nursing home elevated its grade in this category by offering more care from registered nurses to its residents than the average facility. Finally, we considered the percentage of patients who returned to the community from this nursing home. It outperformed most nursing homes in the nation with 49.9 percent of its patients returning home. This is a higher rate than most facilities.

Our inspection scores account for several factors included in a nursing home's inspection report. One of the most important criteria we weigh heavily is the number and severity of deficiencies. This facility was assessed 4 deficiencies on its inspection report, but we were relieved to see that none of the deficiencies were considered to be severe. This means that CMS didn't consider any of the deficiencies to pose an imminent risk to patient health or safety. A couple of minor deficiencies should not lead you to cross a nursing home off your list.

The final category we scored was long-term care, turned out to be this facility's worst area. We gave this facility a D in that area. If you are looking for services other than short-term rehabilitation, you should take a close look at each nursing home's long-term care grades. After looking at the volume of care provided by aids and other staff, we next considered the nursing home's vaccination statistics. This nursing home vaccinated 100 percent of its patients against pneumonia, which is more than the average nursing home. To our surprise, this facility also keeps its residents out of the hospital. Indeed, it had only 1.11 hospitalizations per 1,000 long-term resident days. Unfortunately, some of its other scores in the category were not as favorable as these.
