Facility Inspections
Although we gave this nursing home a terrible overall grade, we awarded it a B+ for our inspections rating. This was due to the nursing home receiving a quality government inspection report this year. Perhaps the most critical factor we consider in computing our inspection grades is deficiencies. Deficiencies can be found on a nursing home's inspection reports. It is generally in your best interest to avoid nursing homes that have too many severe deficiencies. Amazingly, this was one of the few nursing homes in the country that had no deficiencies whatsoever on its inspection report. This is very impressive. The lack of deficiencies proved to be this place's best achievement.
Short-term Care Quality
Turning to the category of short-term care, this facility received just a D in this category. Unfortunately, this proved to be one of its better features. In calculating our short-term care scores, we look at the facility's skilled nursing services, including those performed by registered nurses, respiratory therapists, physical therapists and other types of skilled professionals. Our purpose is to formulate a measure for sizing up the rehabilitation services of various nursing homes. With a grade of this caliber for this category, we were not stunned to learn that this place was below average in many staffing metrics we assess. In fact, it offered significantly fewer physical therapist and registered nurse hours per resident than most facilities. Lastly, we looked at the percentage of residents who were able to return to the community from this nursing home. We found that just 5 percent of this nursing home's patients returned home.

Our third category we assessed is long-term care, where this nursing home received an F. Nursing homes that receive this type of score in long-term care may not provide the type of consistent 24/7 care that some other nursing homes offer. In addition to considering the amount of care provided by nurses and other staff, we also looked at the percentage of residents vaccinated against pneumonia. We discovered that this nursing home administered the vaccine to a relatively low 87.65432 percent of its patients, which is slightly concerning. Surprisingly, this place actually fared well at keeping its residents out of the hospital. In fact, it had only 0.02 hospitalizations per one thousand long-term resident days. This was its best score in this category.
Nurse Quality
The next category we looked at was nursing, where this facility received a very poor grade in this category as well. It received an F in this area, which is obviously very concerning. Our nursing score is based on the nursing home's nurse staffing levels. We factor in both the levels of training of those nurses and the quantity of hours spent with residents. This nursing home provides 0.3 hours of nursing care per resident per day. This is a much lower figure than we are used to seeing. In addition to performing poorly in terms of nursing hours, this facility also had rock bottom marks in the quality-based metrics we looked at in determining our nursing scores. For starters, we looked at the percentage of residents experiencing pressure ulcers and major falls. This facility was at more than twice the national average in both of these data points.
