Facility Inspections
While we gave this nursing home a terrible overall score, we gave it a grade of B+ for our inspections category. This is due to the nursing home receiving a quality government inspection report this year. These inspection ratings weigh several factors, including deficiencies and federal fines. You can learn more about each of these issues by reviewing copies of nursing homes' inspection reports. Fortunately, although this place had a few minor dings on its inspection report, it had no severe deficiencies. Severe deficiencies are ones labeled as categories G through L. This means the inspectors didn't consider any of the deficiencies on this facility's report to pose an imminent threat to patient safety or health. A couple minor deficiencies aren't necessarily the end of the world.
Long-term Care Quality
Moving on to the category of long-term care, this facility was given just a D in this category. As it turns out, this wound up being one of its better grades. In a long-term care setting, the nursing home's primary goal is to keep patients healthy and safe. This is different than skilled nursing or short-term care where the goal is to rehabilitate patients. In addition to assessing the amount of care provided by aids and other staff, we looked at the percentage of residents vaccinated against pneumonia. This nursing home provided the vaccine to 94.666664 percent of its residents, which is more than the vast majority of nursing homes. Surprisingly, this facility also fared well at keeping its residents out of the hospital. In fact, it had just 0.02 hospitalizations per 1,000 long-term resident days. Sadly, a few of its other scores in the category were not as favorable as these.

Nurse Quality
We gave this facility a grade of just D for our nursing rating. We assessed the qualifications of nurses working for the facility, in addition to the number of hours those nurses spent with residents, in determining our grade in this category. This place provided just 0.4 hours of nursing care per patient daily. This is well below average. Lastly, we also assessed some nursing quality-based metrics in computing our nursing grades. This place fared well in terms of avoiding major falls and pressure ulcers. We consider these metrics to be indicators of the quality of nursing care.
Short-term Care Quality
The final category we scored was short-term care. This nursing home was given a very poor grade of F in this area. In the category of short-term care, we endeavor to qualify measures of a facility's rehabilitation services. We look at a nursing home's skilled nursing services, such as those performed by registered nurses and physical, occupational and other therapists. Based on its grade in this area, we weren't stunned to find that this nursing home was well below average in terms of the quantity of registered nurse and physical therapy hours offered to its residents based on the figures we assessed. Frankly, this was about what we expected here. Lastly, we assessed the percentage of residents who were able to return to the community from this nursing home. We found that just 0 percent of this facility's patients returned home.
