Nurse Quality
Although we gave this facility a terrible overall grade, we awarded it a grade of B- for our nursing rating. Our nursing score weighs a number of factors. The most heavily weighted one is the quantity of hours nurses spent with patients. This place provides 0.4 hours of nursing care per patient per day. This is a much lower figure than we were hoping for. Finally, although this place got docked a bit for its nursing hours per resident, it actually excelled in some of our quality-based measures. With fewer than five percent of its residents experiencing pressure ulcers, it performed remarkably well in this statistic.
Short-term Care Quality
Remarkably, we gave this facility a grade of C for our short-term care score, which really isn't too bad of a score. Our short-term care grades are based on the nursing home's quantity of highly-skilled nursing services. This includes a broad range of services, spanning from registered nurses to physical and occupational therapists, not to mention other types of therapy. In this place's case, we were surprised to learn that it actually provides less physical therapist and registered nurse hours per resident than a typical facility. Finally, we considered the percentage of patients who were able to return to the community from this nursing home. We found that 4 percent of this facility's patients returned home. At most nursing homes, closer to half of their patients are able to return home so this figure is below average.

Long-term Care Quality
Sadly, this facility only earned a grade of D for its long-term care rating, which is not one of our better scores. In a long-term care environment, the primary goal is to keep patients healthy and safe. This is different than skilled nursing or short-term care where the goal is to rehabilitate patients. On top of looking at the quantity of care provided by nurses and other staff, we also looked at the percentage of residents vaccinated against pneumonia. This nursing home provided the vaccine to 100 percent of its patients, which is better than the vast majority of nursing homes. To our surprise, this place was able to limit hospitalizations. With just 0.04 hospitalizations per 1,000 long-term resident days, this facility had less hospitalizations than the average nursing home. Unfortunately, some of its other scores in this category weren't as favorable as these.
Facility Inspections
Moving on our final area of inspections, this facility received an F in this area. For facilities that received this poor of an inspection rating, we recommend scrutinizing any severe deficiencies on its inspection report. This particular facility was hit with 7 deficiencies by CMS. The only positive thing we can say is that none of its deficiencies were in the categories that indicate they caused a threat to patient health or safety. Lastly, this nursing home was cited by CMS for possible abuse or neglect. This is generally a really bad sign. We would never recommend this facility.
