Facility Inspections
Although we gave this facility a terrible overall score, we awarded it a grade of B for our inspections rating. This was the result of the facility receiving a quality government inspection report this year. Our inspection grades account for a host of factors included in a nursing home's inspection reports. One of the most important criteria we weigh heavily is the quantity and severity of deficiencies. You generally want to avoid nursing homes with too many deficiencies flagged. Fortunately, although this facility had a few minor dings on its inspection report, it had no severe deficiencies. Severe deficiencies are found in categories G through L. This means CMS didn't consider any of the deficiencies on this facility's report to pose an imminent threat to patient safety or health. A few minor deficiencies are not necessarily the end of the world.
Long-term Care Quality
This facility's second most respectable category is long-term care, but even there it earned just a D in that category. This still happens to be a well below average score. Nursing homes that receive this kind of score in long-term care likely don't provide the type of consistent 24/7 care that some other facilities offer. One of the factors we considered on top of nursing hours is vaccinations. Frankly, we were a bit concerned this nursing home provided the pneumonia vaccination to just 62.171055 percent of its residents. To our surprise, this facility actually fared well at keeping its patients out of the hospital. It had only 0.03 hospitalizations per 1,000 long-term resident days. This was its best score in this category.

Nurse Quality
We also wanted to point out that this nursing home received an F for its nursing grade. Our nursing score focuses on the facility's level of nurse staffing. We weigh both the levels of training of the nurses as well as the number of hours spent with residents. This facility offered a meager 0.4 hours of nursing care per resident daily. This is a very low total compared to most nursing homes. Lastly, we also looked at some nursing quality measures in computing our nursing scores. This nursing home fared well in terms of minimizing its residents' major falls and pressure ulcers. Many consider these metrics to be reliable indicators of the quality of nursing care.
Short-term Care Quality
The final category we rated is short-term care. This facility was given a poor grade of F in that category. In the area of short-term care, we try to evaluate indicators of a facility's rehabilitation services. We look at the facility's skilled nursing services, including those performed by registered nurses and physical and occupational therapists. With a score of this caliber for this category, we weren't shocked to learn that this facility was well below average in several staffing figures we looked at. The facility supplied substantially fewer physical therapist and registered nurse hours per patient than most nursing homes. Finally, we considered the percentage of residents who were able to return to the community from this facility. This wasn't a strength for this nursing home. We found that just 4.3 percent of this nursing home's patients returned home. At most nursing homes, about half of their patients are able to return home so this is well below average.
