Facility Inspections
On top of performing well in its overall grade, this facility performed at an even higher level in the area of inspections. In fact, it received a nearly flawless inspection report this year. As a result, it received one of our highest grades in that category with a score of A+. We take several aspects of a facility's inspection report into consideration in computing our inspection grades. One key factor is health deficiencies. You should keep in mind that the severity of deficiencies is arguably more meaningful than the number of deficiencies, as some of these are relatively minor. Fortunately, although this facility had some minor dings on its report, it had zero severe deficiencies. Severe deficiencies are found in categories G, H, I, J, K and L. This means the inspectors didn't consider any of the deficiencies on this facility's report to pose an imminent threat to patient safety or health. Finally, this facility had no substantiated complaints this year from residents, which is an excellent sign.
Long-term Care Quality
This facility also was awarded a strong long-term care grade. In fact, we gave it an above average score of B+ in this area. When facilities receive a score in this range in this category it generally means it has plenty of staff and is a quality place to live on a permanent basis. In addition to considering the quantity of care provided by nurses aids and other staff, we looked at the number of residents vaccinated against pneumonia. This nursing home administered the vaccine to 100 percent of its patients, which is what we like to see. Pneumonia is too frequently a life or death health condition for nursing home residents so we strongly prefer when a facility does not take any chances. Lastly, this facility was able to limit hospitalizations. In fact, it had only 0.01 hospitalizations per one thousand long-term resident days, which is an impressively low figure.

Short-term Care Quality
The next area we looked at was short-term care, where this nursing home was awarded a grade of B- in this area. In the area of short-term care, we attempt to assess indicators of a nursing home's rehabilitation services. We look at a facility's skilled nursing services, including the ones performed by registered nurses and physical and occupational therapists. Unfortunately, we weren't able to find information on registered nurse or physical therapy staffing for this nursing home. The final statistic we looked at in this category is the number of residents that who were able to eventually return home from the facility. We found that just 5.6 percent of this nursing home's residents returned home.
Nurse Quality
The final category we assessed was nursing. We awarded this facility a grade of D in our nursing category, leaving this as the nursing home's weakest grade. In computing a nursing home's nursing rating, we assess the amount of time nurses are seeing residents and the skill levels of those nurses. This nursing home provides 0.3 hours of nursing care per resident per day. This is a much lower figure than we are used to seeing. Lastly, we also assessed some nursing quality-based metrics in determining our nursing grades. This facility fared well in terms of minimizing its residents' pressure ulcers and major falls.
