Grade: A

While we were not high on this facility's overall score, it actually received a very impressive inspection report this year. In fact, it received an excellent inspection report this year. As a result, it received one of our best scores in that category with an A. Our inspection scores weigh several factors found on a facility's inspection reports. One of the most important criteria we weigh heavily is the number and severity of deficiencies. You generally want to avoid places with too many severe deficiencies flagged. While this facility had some minor dings on its report, it had no severe deficiencies. Severe deficiencies are those labeled as categories G through L. This tells you the government inspectors did not consider any of the deficiencies on this facility's report to pose an imminent threat to patient health or safety. A few minor deficiencies are not the end of the world.

Long-term Care Quality

Grade: B-minus

This facilities next most favorable area was long-term care. We awarded it a B- in this area. This is essentially a middle of the road score in this category. For prospective residents seeking a permanent place to live as opposed to rehabilitation, long-term care is a very important category. Once we assessed the volume of care provided by nurses and other staff, we next considered the nursing home's vaccination data. We were pleased to find that this nursing home vaccinated 100 percent of its patients for pneumonia. This facility also excelled at keeping its patients out of the hospital. It had only 0.01 hospitalizations per one thousand long-term resident days, which is a remarkably low number.

Nurse Quality

Grade: F

We also wanted to draw your attention to this facility's poor nursing grade where it received an F. Our nursing score includes many components, many of which are based on nursing hours. This nursing home offered a meager 0.3 hours of nursing care per resident each day. Finally, we also looked at some nursing quality measures in calculating our nursing ratings. This place performed well in the area of avoiding pressure ulcers and major falls.

Short-term Care Quality

Grade: F

The final area we rated was short-term care, where this facility received a rock bottom score in this area also. Unfortunately, we gave it an F for this area, which is obviously very concerning. Short-term care grades are often employed to judge a nursing home's rehabilitation services. To have highly graded rehabilitation services, facilities generally need to have better levels of highly skilled nursing services. Skilled nursing includes registered nurses, physical therapists, speech therapists and other licensed professionals. Not surprisingly, we found that this nursing home provides far less physical therapist and registered nurse hours per resident than the average facility. The final datapoint we considered in this category is the number of residents that who were able to eventually return home from the facility. This nursing home struggled quite a bit in this area as well, with just 3.8 percent of its residents returning home.
