Facility Inspections
While this nursing home's overall grade was not a highlight, it actually really excelled in the category of inspections. In fact, we awarded it an A- for that category, which is one of our highest scores. Inspection grades take several factors into consideration, including deficiencies, substantiated complaints and federal fines. You can find more information about each of these factors by reviewing copies of nursing homes' inspection reports. This facility was hit with 5 deficiencies on its inspection report, but none were considered to be severe. This means that CMS did not deem any of these deficiencies to cause an imminent threat to resident safety or health. A few minor deficiencies aren't necessarily the end of the world.
Short-term Care Quality
We gave this facility just a D for our short-term care rating and that actually ended up being its second best category. Our short-term care grades are probably more meaningful for patients in need of a nursing home for rehabilitation. Rehabilitation usually requires additional skilled nursing services. This includes not just nursing services, but also physical and respiratory therapy, as well as other types of therapy. Considering its grade in this area, we were not surprised to find that this nursing home was well below average in terms of its quantity of physical therapy and registered nurse hours offered to its patients based on the data we assessed. Frankly, this was what we expected to find. Lastly, we looked at the percentage of residents who were able to return to the community from this facility. We found that just 5 percent of this nursing home's patients returned home.

Nurse Quality
We were stunned to find that this facility also excelled in nursing. We gave it an D for that category, which is one of our best grades. There are many criteria included in this category. Many of these factors are tied to levels of nurse staffing. This nursing home offered only 0.3 hours of nursing care per resident on a daily basis. Lastly, we also looked at some nursing quality-based metrics in determining our nursing grades. This nursing home fared well in the area of minimizing its residents' major falls and pressure ulcers. We consider these areas to be measures of the quality of nursing care.
Long-term Care Quality
The final category we rated is long-term care. This nursing home was given a poor grade of F in this category. If you are seeking anything other than rehabilitation, you should take a close look at each facility's long-term care scores. On top of looking at the volume of care provided by nurses aids and other staff, we looked at the number of residents vaccinated for pneumonia. This nursing home vaccinated 96.91876 percent of its patients, which is better than the average nursing home. To our surprise, this place was able to limit hospitalizations. With just 0.01 hospitalizations per one thousand long-term resident days, this facility has fewer hospitalizations than most nursing homes. Sadly, some of its other scores in the category were not as favorable as these.
