Facility Inspections
While this nursing home's overall grade was not a highlight, it actually really excelled in the category of inspections. In fact, we awarded it a grade of A for that category, which is one of our best scores. Perhaps the most critical factor we consider in computing our inspection ratings is deficiencies. These deficiencies can be found on a nursing home's inspection reports. It is generally best to avoid facilities that had too many deficiencies. Fortunately, although this nursing home had some minor dings on its report, it had no severe deficiencies. Severe deficiencies are labeled as categories G, H, I, J, K and L. This means that the government inspectors did not consider any of the deficiencies on this facility's report to pose an imminent threat to patient safety or health. A couple minor deficiencies are not the end of the world.
Long-term Care Quality
Sadly, this facility only earned a grade of D for its long-term care rating, which ended up being one of its more respectable category scores. In a long-term care setting, the primary objective is to keep patients as healthy and safe as possible. This is different than skilled nursing or short-term care where the goal is to rehabilitate residents. On top of assessing the volume of care provided by nurses and other staff, we also looked at the percentage of residents vaccinated against pneumonia. This facility vaccinated 98.0456 percent of its residents, which is more than the majority of nursing homes. Surprisingly, this nursing home also fared well at keeping its residents out of the hospital. In fact, it had only 0.02 hospitalizations per 1,000 long-term resident days. Sadly, a few of its other scores in the category were not as favorable as these.

Short-term Care Quality
Unfortunately, this facility received an F for its short-term care grade. Our short-term care grade is typically used to measure a facility's rehabilitation services In order to offer highly graded rehabilitation services, facilities generally must offer better levels of highly skilled nursing services. These types of services include registered nurses, physical therapists, occupational therapists and other highly trained professionals. Considering its score in this area, we were not stunned to learn that this place is well below average in terms of its number of registered nurse and physical therapy hours provided to its patients based on the metrics we assessed. Honestly, this was what we expected here. Finally, we looked at the percentage of patients that who were able to eventually return home from this nursing home. This nursing home didn't fare well here either. In fact, we found that just 0 percent of this facility's patients were able to return home. This figure was quite a bit off the national average.
Nurse Quality
The final category we looked at is nursing. This facility was given a very poor grade of F in this area. Our nursing grade weighs a host of factors. The most important factor is the number of hours nurses spend with residents. This nursing home offered just 0.3 hours of nursing care per resident daily. This place also had abysmal marks in some of the quality-based measures to go along with its low nursing hour totals. We looked at the percentage of residents suffering pressure ulcers and we were disappointed. This nursing home was at roughly 150 percent of the national average in this measure. This is a bad sign when you consider that so many pressure ulcers are preventable with better nursing care. This metric helped pull down this facility's nursing grade quite a bit.
