This grade is frequently a fair assessment of a facility's rehabilitation services. Fortunately, it looks like this facility employs registered nurses. Not all nursing homes employs these types of nurses. However, based on the data this nursing home provided, it does not look like the facility employs physical therapists. Lastly, we looked at the percentage of residents who eventually returned home from this facility. We found that 5.9 percent of this facility's residents returned home.

Nurse Quality

Grade: B

Another one of this nursing home's stronger areas was in the category of nursing care. In that area, we gave this facility a grade of B. Finally, our nursing grades also factor in quality-based assessments, such as avoiding major falls. This nursing home performed well in this area. Avoiding major falls is typically a good indicator that a nursing home has reliable quality controls in place. Major falls can generally be avoided if more nurses aids and better safety protocols are in place.

Long-term Care Quality

Grade: D

Moving on to the category of long-term care, this nursing home was awarded a grade of just D in that area. In a long-term care environment, the primary objective is to maintain residents' quality of life and keep them safe. Once we assessed the volume of care provided by nurses aids and other staff, we then analyzed the nursing home's vaccination records. This facility vaccinated 95.419846 percent of its residents against pneumonia, which is better than the majority of nursing homes. Surprisingly, this place was able to keep its residents out of the hospital. Indeed, it had only 0 hospitalizations per one thousand long-term resident days. Unfortunately, some of its other scores in this category weren't as favorable as these.

Facility Inspections

Grade: F

The last category we assessed is inspections. This facility did not receive favorable government inspection reports in recent years. We gave it an F in this category. This ended up pulling down this nursing home's overall grade quite a bit. You should pay close attention to any deficiencies on a facility's inspection reports when it receives this poor of an inspection score. We would pay close attention to the level of severity of those deficiencies. This facility was assessed 6 deficiencies by government inspectors. The only favorable thing we found is that none of its deficiencies were in the categories that indicate that they caused a threat to patient health or safety. Lastly, we also want to draw your attention to the fact that this facility received significant government fines of more than $100,000. This often suggests that a nursing home's poor performance spanned several years.
