Facility Inspections
While this nursing home's overall grade was as bad as it gets, it actually excelled in the category of inspections. In fact, we awarded it an A- for that category, which is one of our highest scores. Our inspection scores are based on several pieces of information found on the a nursing home's recent inspection reports. Deficiencies are a key thing to look for on these reports. You should especially avoid facilities that have severe deficiencies associated with risks to resident well being. While this nursing home had some deficiencies on its government inspection report, none of them were major deficiencies based on CMS' scale. A few minor deficiencies are not necessarily the end of the world.
Short-term Care Quality
Included in this nursing home's litany of terrible grades was its F in the category of short-term care. Our short-term care grade is generally employed to measure a nursing home's performance with rehabilitation. To provide highly graded rehabilitation services, facilities generally must feature higher levels of highly skilled nursing. These types of services include registered nurses, physical therapists, speech therapists and other highly skilled individuals. Based on its poor rating in this category, we were not stunned to learn that this place provides significantly fewer physical therapist and registered nurse hours per resident than most other facilities. Lastly, we looked at the percentage of residents who were able to return to the community from this facility. We found that just 4 percent of this facility's patients returned home.

Nurse Quality
Unfortunately, we gave this facility an F for its nursing grade. Our nursing grade assesses several subcategories, but the main one is the number of nurse hours spent with patients. This particular facility provided a meager 0.4 hours of nursing care per resident per day. This figure is significantly below average. Lastly, we also looked at some nursing quality-based metrics in computing our nursing ratings. This facility fared well when it comes to minimizing its residents' major falls and pressure ulcers.
Long-term Care Quality
The last category we rated was long-term care, where this facility was given a rock bottom grade here also. Sadly, it received an abysmal F for this category, which is obviously a major disappointment. For patients looking for a permanent residence rather than rehabilitation, long-term care grades are an important measure. In addition to assessing the quantity of care provided by nurses aids and other staff, we looked at the percentage of residents vaccinated against pneumonia. This nursing home gave the vaccine to 95.28302 percent of its patients. To our surprise, this place also fared well at keeping its patients out of the hospital. In fact, it had only 0.02 hospitalizations per one thousand long-term resident days. Sadly, some of its other scores in this category weren't as favorable as these.
