Facility Inspections
While we graded this place incredibly poorly overall, it actually earned a fairly impressive government inspection report this year. As a result, it earned one of our better grades in that category with a grade of B+. Arguably the most significant factor we consider in calculating our inspection grades is deficiencies. These deficiencies can be found on a facility's inspection reports. It is generally best to avoid nursing homes that had too many severe deficiencies. This nursing home received 4 deficiencies on its inspection report, but we were relieved to see that none were considered severe. This means that the government inspectors didn't deem any of the deficiencies to cause an imminent threat to resident health or safety. A few minor deficiencies aren't necessarily the end of the world.
Nurse Quality
Unfortunately, this facility only received a grade of D for its nursing grade, which was actually one of its better category grades. When determining our nursing scores, we analyze both nursing hours and the skill levels of those nurses. With a meager 0.3 hours of nursing care per patient per day, this nursing home's nurse staffing levels were significantly below the national average. Lastly, we also looked at some nursing quality measures in determining our nursing grades. This place fared well when it comes to minimizing its residents' major falls and pressure ulcers.

Long-term Care Quality
This facility wound up receiving a rock-bottom score in our long-term care category. It received an F in this category. Facilities that receive this kind of grade in long-term care may not provide the type of consistent 24/7 care that some other facilities offer. On top of considering the volume of care provided by nurses aids and other staff, we looked at the number of residents vaccinated against pneumonia. This facility provided the vaccine to 90.90909 percent of its patients. This is slightly less than we were hoping for but still a reasonably acceptable percentage. Surprisingly, this facility was actually decent at keeping its residents out of the hospital. It had just 0.01 hospitalizations per 1,000 long-term resident days. This was its best feature in this category.
Short-term Care Quality
Moving on to the last category, this facility really didn't perform very well in this area either. With a very poor grade of F in short-term care, this is about as terrible as it gets. Our short-term care ratings are probably most meaningful for people needing rehabilitation from their nursing home. Rehabilitation typically mandates higher levels of skilled nursing services. Skilled nursing includes not just nursing, but also physical and speech therapy, as well as other forms of therapy. Considering its grade in this area, we were not surprised to discover that this nursing home was well below average in terms of the number of registered nurse and physical therapy hours offered to its patients based on the figures we looked at. Lastly, we assessed the number of residents who were able to return to the community from this facility. This definitely was not a strength for this facility. We found that just 4 percent of this nursing home's residents returned home. At most facilities, around half of their residents are able to return home so this is well below average.
