Scottsdale Village Square is a large nursing home located in Scottsdale, Arizona. This is a lower end facility, with an overall grade of D. Not surprisingly, this is one of bottom three nursing homes in Scottsdale. With plenty of other nursing homes in the city, you should be able to do better. While we gave this nursing home a poor overall rating, we gave it a fairly respectable score in the category of inspections, where it received a grade of B-. This was better than its other categories. We believe that one of the most critical factors in evaluating an inspection report is deficiencies. We especially focus on the severity of the deficiencies. This place received 4 deficiencies on its inspection report, but we were relieved to see that none of the deficiencies were considered to be major deficiencies. This means that CMS did not consider any of the deficiencies to pose an immediate threat to patient health or safety. Finally, CMS flagged this nursing home for possible abuse or neglect. This is not what you want to see. We advise you to direct your search elsewhere.

One of the other reasons we graded this facility so highly is that it earned a very impressive nursing grade. This proved to be its second best category grade. In that area, we awarded this nursing home an B-. The nursing grade weighs quite a few datapoints, but the main one is the level of nurse hours per patient per week. With 6 hours of nursing care per resident each day, this place surpassed the national average. We also look at the skill levels of nurses and apply more weight to hours worked by skilled nurses. Finally, we also looked at a few quality-based metrics in determining our nursing ratings. This place was above average in several of the major statistics we assessed, with good scores for minimizing its residents' major falls and pressure ulcers.

This nursing home was awarded its next best category grade in the area of short-term care. We gave this facility a grade of B- in this category, which is a respectable grade. With our short-term care score, we strive to forge a sound gauge for rehabilitation. In doing so, we assess a facility's scope of highly-skilled nursing services, including both registered nurses and physical therapy, as well as occupational therapy. In this nursing home's case, we were surprised to learn that it actually provides less physical therapist and registered nurse hours per patient than a typical facility. The final datapoint we looked at in this category is the percentage of patients who were able to leave the facility and return to the community. This proved to be a stronger area than its staffing metrics. This place performed well in this metric with 67.5 percent of its residents returning home.
