Facility Inspections
Although we gave this facility a terrible overall grade, we awarded it a C for our inspections rating. In fact, the facility received very impressive inspection report this year. We would argue that one of the most critical factors in assessing an inspection report is deficiencies. We especially look at the severity of the deficiencies. This nursing home had 4 deficiencies on its government inspection report, but we were relieved to find that none were in any of the categories labeled G through L. This indicates that this facility dodged the more significant classes of deficiencies. Finally, this facility was cited by CMS for possible abuse or neglect. We would not recommend this facility under any circumstance.
Long-term Care Quality
Unfortunately, this facility's next highest category was long-term care. Nevertheless, with a D in this category, it still performed relatively poorly. In a long-term care environment, the facility's primary objective is to keep patients healthy and safe. This is different than skilled nursing or short-term care where the goal is to rehabilitate patients. In addition to looking at the quantity of care provided by nurses aids and other staff, we analyzed the number of residents vaccinated against pneumonia. Thankfully, this nursing home vaccinated 94.53376 percent of its residents, which is a very respectable figure. To our surprise, this facility also keeps its patients out of the hospital. In fact, it had only 0.02 hospitalizations per one thousand long-term resident days. Unfortunately, some of its other scores in the category were not as favorable as these.

Short-term Care Quality
This facility's second lowest category is short-term care, where it received just a D in that area. This is a relatively poor score. Short-term care ratings are based in part on a nursing home's quantity of skilled highly skilled professionals. This includes a wide spectrum of nursing services, spanning from registered nurses to physical and speech therapists, as well as other forms of therapy. With its grade in this area, we weren't stunned to discover that this nursing home was well below average in terms of the number of registered nurse and physical therapy hours offered to its residents based on the data we looked at. Honestly, this was about what we expected to find. Finally, we assessed the percentage of residents who were able to return to the community from this nursing home. This definitely wasn't a strength for this facility. We found that just 4.9 percent of this facility's residents returned home. At most nursing homes, about half of their residents are able to return home so this is well below average.
Nurse Quality
Moving on to our last category, we awarded this facility a D in our nursing category. This was the nursing home's weakest category rating. Nursing scores are based largely on nurse staffing levels. This particular nursing home provided a meager 0.3 hours of nursing care per patient each day. This number is well below the national average. In addition to its weak nursing hour totals, this place earned poor scores in some of the quality-based measures we looked at in determining our nursing grades. We found that a higher percentage of this facility's patients sustained falls which led to serious injury. We believe that many falls could likely be avoided with better nursing care. This metric contributed to this place receiving a terrible nursing score.
