Sante of Surprise is a small nursing home located in Surprise, Arizona. It looks like this nursing home is among the highest-rated facilities we assessed. A grade of this caliber requires superb scores across the board. We can't find many bad things to say about this place. Its scores are impeccable. Headlining this place's remarkable report card is its inspection reports, which is addressed in the next paragraph.

Facility Inspections
One of the many reasons this turned out to be a quality nursing home is that it received an A+ inspection grade. This is simply as good as it gets in this critical category. Inspection scores are tied to several pieces of information located in the a nursing home's recent inspection reports. Nursing homes that excel in this area typically have few deficiencies on their reports. Most importantly, these places should not have any severe deficiencies which are associated with endangerment of patients. It doesn't appear that we have deficiency data for this facility. Finally, this facility had no substantiated complaints this year from residents, which is an excellent sign.
Short-term Care Quality
Adding to its dominant profile, this nursing home also performed well in our short-term care category. In fact, it received a score of A in the area. In the category of short-term care, we try to evaluate measures of a nursing home's rehabilitation services. We assess a nursing home's skilled nursing services, including the ones performed by registered nurses and physical, occupational and other therapists. This nursing home provides more services with physical therapists and registered nurses than most facilities we looked at. Lastly, we looked at the percentage of patients who were able to return to the community from this facility. We found that it outperformed the vast majority of facilities in the nation in this area with 73.5 percent of its residents returning home.
Long-term Care Quality
This facility also was elite in the category of long-term care, where we gave it a grade of A-. Very few nursing homes received a better grade in this category. If you are seeking anything other than rehabilitation, you should take a close look at each nursing home's long-term care grades. On top of assessing the amount of care provided by nurses aids and other staff, we analyzed the number of residents vaccinated against pneumonia. We were not able to locate this facility's vaccination rates. Clearly, this facility is doing something right in this area as it also excelled at keeping its residents out of the hospital. In fact, it had only 0 hospitalizations per one thousand long-term resident days, which is a remarkably low figure.
Nurse Quality
The last area we assessed was nursing, where this facility received a B. While this turned out to be its poorest category rating, this is still a strong rating. There are many criteria within this grade. Most of the data points relate to the quantity of nurse staffing. This place provided 4.4 hours of nursing care per patient on a daily basis, which is among the highest figures in the country. A significant percentage of those hours were provided by registered nurses, which are one of the most skilled levels of nurses. We are very impressed by these statistics. On top of offering impressive levels of nursing care, this nursing home also excelled in the quality-based metrics we assessed. By way of illustration, it performed as well as any nursing home in the state in the area of avoiding pressure ulcers and major falls. We view these areas as good indicators of the quality of nursing care provided.
