Long-term Care Quality
Although we gave this facility a terrible overall grade, we awarded it a grade of C for our long-term care rating. For prospective residents seeking a permanent place to live rather than rehabilitation, long-term care is an important category. One of the statistics we considered in addition to nurse's aid hours is vaccinations. Fortunately, this nursing home administered the pneumonia vaccine to 98.8024 percent of its patients. Finally, this nursing home was also able to limit hospitalizations. It had only 0.02 hospitalizations per one thousand long-term resident days, which is a remarkably low figure.
Short-term Care Quality
This facility ended up receiving a rock-bottom grade in our short-term care area. It received a lowly F in this area. In the category of short-term care, we strive to assess indicators of a facility's rehabilitation services. We assess the facility's skilled nursing services, including those performed by registered nurses and physical therapists. Given its abysmal score in this category, we weren't surprised to find that this nursing home offers far fewer physical therapist and registered nurse hours per patient than most nursing homes. The final datapoint we assessed in this area is the percentage of residents who ultimately were able to return home from the facility. This nursing home struggled quite a bit in this area as well, with just 4.6 percent of its patients returning home. Unfortunately, this was well below the national average.

Facility Inspections
Among this facility's alarming category grades was an F for its inspection grade. More often than not, this means we found some red flags on the facility's inspection reports. For places with bad inspection scores, you should really scrutinize any severe deficiencies found on their inspection reports. This facility had 2 deficiencies by government inspectors. The only favorable thing we can say is that none of its deficiencies were in the categories that suggest they caused a risk to patient health or safety. Finally, this facility was cited by CMS for possible abuse or neglect. We would not recommend this facility under any circumstance.
Nurse Quality
The final category we assessed was nursing. This nursing home received an abysmal grade of F in this area. Nursing grades are largely associated with a facility's nurse staffing. This facility offered only 0.4 hours of nursing care per patient daily. Unfortunately, this facility also had abysmal marks in several of the quality-based measures to pair with its low nursing hour totals. For starters, we looked at the percentage of patients experiencing pressure ulcers. This nursing home was at more than double the national average in this datapoint.
