Pine Haven Home is the sole option we found in Philmont, New York. Sporting an overall grade of D, this is a lower end nursing home. This wasn't the worse nursing home we found, but we'd still be very reluctant to recommend it. Facility Inspections
While we were not high on this facility overall, we awarded it a grade of A for our inspections rating. This score is far more impressive than the nursing home's overall score. Perhaps the most critical factor we look at in determining our inspection ratings is deficiencies. Deficiencies are found on a facility's recent inspection reports. It is generally best to avoid places that had a long list of severe deficiencies. While this place had some deficiencies on its inspection report, none of them were severe based on CMS' scale. A few minor deficiencies aren't necessarily the end of the world.
Long-term Care Quality
This facilities second most favorable area was long-term care. We awarded it a grade of B- in this category. This is essentially a middle of the road score in this category. In crafting our long-term care ratings, we assess the facility's personal care services. On top of assessing the volume of care provided by nurses aids and other staff, we looked at the number of residents vaccinated for pneumonia. This nursing home provided the vaccine to 100 percent of its residents. Finally, we looked at the facility's number of hospitalizations. Here we found that this nursing home had 3.42 hospitalizations per 1,000 long-term resident days. Although this figure is quite a bit higher than the national average, this number may be skewed for some facilities due to the preexisting medical conditions of patients.
Short-term Care Quality
Moving on to the category of short-term care, this facility received just a D in this category. Our short-term care ratings are based on a nursing home's quantity of skilled skilled healthcare professionals. This includes a wide range of nursing services, ranging from registered nurses to physical and respiratory therapists, not to mention other types of therapy. Given its poor grade, we were not shocked to find that this nursing home provided substantially fewer registered nurse hours per resident than most other facilities. Lastly, we assessed the number of patients who were able to return to the community from this nursing home. This definitely was not a strength for this facility. We found that just 36.8 percent of this facility's patients returned home. At most facilities, around half of their patients are able to return home so this is well below average.
Nurse Quality
The next area we rated was nursing. We gave it an abysmal F in this category, which is a bottom of the barrel grade. Our nursing rating looks at the facility's nurse staffing levels. We look at both the levels of training of those nurses as well as the amount of time spent with residents. This place provided a meager 3.2 hours of nursing care per patient each day. This is not a very impressive figure. To pair with its subpar totals in the area of nursing hours per resident, this facility also did not perform as well in the quality-based measures we looked at in determining our nursing ratings. We looked at the percent of patients experiencing major falls and pressure ulcers. This place had more falls and pressure ulcers than the average nursing home. This could be a bad sign when you consider that many bed sores and falls are believed to be preventable with better nursing care. Unfortunately, these statistics pulled down this nursing home's nursing grade quite a bit.
