Nurse Quality

Grade: B-plus

Although we gave this facility a terrible overall grade, we awarded it a grade of B+ for our nursing rating. In computing our nursing grades, we look at both the number of hours nurses spend with residents and the training levels of those nurses. This nursing home provides 0.3 hours of nursing care per resident per day. Finally, we also assessed several nursing quality-based metrics and this place excelled in some of these areas. With less than five percent of its residents sustaining pressure ulcers, it performed as well as any nursing home Iowa in this category. This is generally a good indicator of quality nursing care. Pressure ulcers can generally be avoided by providing better nursing care, such as having a policy of moving residents even once per day.

Short-term Care Quality

Grade: D

As it turns out, this facility only received a D for its short-term care rating, which was actually one of its more favorable category grades. In calculating our short-term care scores, we assess a facility's levels of skilled nursing services, which includes registered nurses, physical therapists occupational therapists and other types of therapists. This rating is frequently a reliable assessment of the facility's ability to rehabilitate patients. With a score in this range in this category, we weren't surprised to learn that this facility was well below average in two of the key staffing metrics we focus on. The nursing home offered far fewer physical therapist and registered nurse hours per resident than most other facilities. Lastly, we considered the percentage of residents who were able to return to the community from this facility. This definitely wasn't a strength for this facility. We found that just 3.6 percent of this facility's patients returned home. At most nursing homes, about half of their residents are able to return home so this is well below average.

Facility Inspections

Grade: F

This facility received an F for its inspection grade. For nursing homes with bad inspection ratings, you should focus on any severe deficiencies found on their recent inspection reports. This particular nursing home had 3 deficiencies on its inspection report. The only positive thing we can say is that none of these deficiencies were in the categories that suggest that they posed a risk to resident health or safety. Finally, we also need to draw your attention to the fact that this nursing home received large government fines in recent years of more than $100,000. This often means that a bad performance spanned multiple years.

Long-term Care Quality

Grade: F

Switching gears to our final area, this nursing home really didn't perform very well in this area either. With a bottom of the barrel grade of F in long-term care, this is about as bad as it gets. When nursing homes receive a grade in this range in long-term care it typically means the nursing home didn't perform well in our quality measures relating to patient care. Once we looked at the amount of care provided by aids and other staff, we turned to the facility's vaccination records. Candidly, we were a bit worried that this nursing home vaccinated 80 percent of its patients for pneumonia, which happens to be several points lower than the national average. To our surprise, this nursing home actually fared well at keeping its patients out of the hospital. Indeed, it had just 0.02 hospitalizations per one thousand long-term resident days. This was its best score in this category.
