Facility Inspections
To go along with its relatively modest overall grade, we gave this facility an A for its inspection grade. This grade is far more impressive than the facility's overall rating. Inspection grades weigh several factors found on a nursing home's inspection reports. One key criteria we consider is the number and severity of deficiencies. Nursing homes with better grades in this area generally have very few of these severe deficiencies. While this place had a few minor dings on its government inspection report, it had zero severe deficiencies. Severe deficiencies are ones found in categories G, H, I, J, K and L. This means the government inspectors didn't deem any of the deficiencies on this facility's report to pose an imminent threat to patient health or safety. A few minor deficiencies should not stop you from considering a nursing home.
Long-term Care Quality
This facility's next highest category turned out to be its long-term care score, where it received a B-. For long-term care residents, the primary goal is to keep patients as healthy and safe as possible. This is different than skilled nursing or short-term care where the goal is to rehabilitate residents. In addition to looking at the amount of care provided by aids and other staff, we also looked at the percentage of residents vaccinated against pneumonia. This nursing home administered the vaccine to 93.82023 percent of its patients. This is slightly lower than we expected but still a reasonably acceptable percentage. On a more positive note, this place was able to limit hospitalizations. It had only 0.04 hospitalizations per one thousand long-term resident days, which is a remarkably low number.

Short-term Care Quality
The next category we graded is short-term care, where this nursing home was given a grade of C in this category. In calculating our short-term care scores, we assess the nursing home's skilled nursing services, including those performed by registered nurses, occupational therapists, physical therapists and other types of therapists. The objective is to create a barometer for comparing the rehabilitation services of various nursing homes. In this facility's case, we were surprised to learn that it actually provides less physical therapist and registered nurse hours per resident than the typical facility. Finally, we assessed the number of residents who were able to return to the community from this facility. We found that 4.8 percent of this facility's patients returned home. At most facilities, closer to half of their patients are able to return home so this figure is below average.
Nurse Quality
The last area we graded was nursing, was this facility's weakest area. We gave this nursing home a grade of D in that area. There are a variety of subcategories within this grade. Many of these criteria reflect levels of nurse staffing. This facility provided just 0.4 hours of nursing care per resident per day. This is a very low total compared to most nursing homes. In addition to performing poorly in terms of nursing hours, this facility also had rock bottom marks in the quality-based metrics we looked at in computing our nursing ratings. For starters, we looked at the percent of residents experiencing pressure ulcers and major falls. This facility was at more than double the national average in both of these statistics.
