Facility Inspections
While we graded this nursing home incredibly poorly overall, it received an impressive government inspection report this year. As a result, it earned one of our best grades in that category with an A-. Arguably the most significant factor we consider in determining our inspection ratings is deficiencies. Deficiencies can be found on a facility's inspection reports. It is generally in your best interest to avoid facilities that have too many deficiencies. Fortunately, although this place had a few minor dings on its government inspection report, it had zero severe deficiencies. Severe deficiencies are those found in categories G through L. This means CMS did not deem any of the deficiencies on this facility's report to pose an imminent threat to patient safety or health. A couple minor deficiencies are not necessarily the end of the world.
Long-term Care Quality
Unfortunately, this facility received an F for its long-term care grade. Nursing homes that receive this type of score in long-term care may not provide the type of consistent around the clock care that some other nursing homes provide. Once we assessed the quantity of care provided by aids and other staff, we then looked at the nursing home's vaccination records. We were a bit alarmed this nursing home vaccinated 72.78107 percent of its residents for pneumonia, which happens to be quite a few points below what we were hoping for. To our surprise, this place was actually able to limit hospitalizations. With just 0.01 hospitalizations per one thousand long-term resident days, this nursing home had fewer hospitalizations than the average nursing home. This is its best feature in this category.

Nurse Quality
Unfortunately, we gave this facility an F for its nursing grade. Our nursing grade is primarily based on a nursing home's level of nurse staffing. With just 0.3 hours of nursing care per patient per day, this place's nurse staffing levels were far below the national average. Lastly, we also looked at some nursing quality-based metrics in determining our nursing ratings. This facility fared well when it comes to minimizing its residents' major falls and pressure ulcers.
Short-term Care Quality
The last category we graded is short-term care, in which this facility was given a bottom of the barrel grade here as well. Sadly, it received an abysmal F in this category, which is obviously somewhat alarming. Our short-term care ratings are based in part on a facility's quantity of skilled skilled healthcare professionals. This means a vast spectrum of nursing services, ranging from registered nurses to physical and occupational therapists, as well as other variations of therapy. Not surprisingly, we found that this nursing home provides far less physical therapist and registered nurse hours per resident than the average facility. Finally, we considered the percentage of patients who were able to return to the community from this nursing home. We found that just 4.2 percent of this facility's patients returned home.
