Facility Inspections
While we were not very complementary of this facility's overall grade, it actually received a very impressive inspection report this year. In fact, it received a nearly flawless inspection report this year. As a result, it received one of our highest grades in that category with an A-. We weigh several aspects of a nursing home's inspection report in determining our inspection ratings. One key factor is health deficiencies. It should be noted that the severity of deficiencies is arguably more meaningful than the quantity of deficiencies, as some end up being relatively insignificant. Although this place had a few deficiencies on its government inspection report, none of them were major deficiencies based on CMS' deficiency scale. A few minor deficiencies are not the end of the world.
Long-term Care Quality
This nursing home received its next highest category score in the category of long-term care. We gave this facility a C in this category, which is a reasonably acceptable grade. Our long-term care grade is generally used to assess a nursing home's performance as a traditional nursing home, as opposed to focusing on the skilled nursing services provided by a rehabilitation facility. In addition to looking at the quantity of care provided by aids and other staff, we also looked at the percentage of residents vaccinated for pneumonia. This nursing home gave the vaccine to 95.454544 percent of its residents. Lastly, this nursing home was able to limit hospitalizations. While it had 1.72 hospitalizations per one thousand long-term resident days, its risk adjusted score was better than the majority of nursing homes due to it having more complicated patients.

Nurse Quality
Unfortunately, this facility only received a grade of D for its nursing grade, which was actually one of its better category grades. Our nursing rating features a number of datapoints. The most important one is the amount of time nurses spend with residents. This facility averaged just 2.7 hours of nursing care per patient on a daily basis. This is not a very impressive figure. Lastly, we also looked at some quality measures in computing our nursing ratings. This nursing home was above average in two of the statistics we considered, with good marks for avoiding major falls and pressure ulcers.
Short-term Care Quality
The final category we graded was short-term care, was this facility's weakest area. We gave this facility a D in this category. In computing our short-term care ratings, we scrutinize a nursing home's levels of skilled nursing services, which includes registered nurses, physical therapists speech therapists and other types of therapists. This category is considered to be a reliable assessment of a facility's rehabilitation. Considering its grade in this category, we were not stunned to learn that this facility was well below average in terms of its number of registered nurse and physical therapy hours provided to its patients based on the measures we assessed. Frankly, this was about what we expected here. Lastly, we considered the number of residents who were able to return to the community from this facility. We were surprised to find that it performed better in this area, with 55.3 percent of its patients returning home. This was just about this nursing home's sole highlight in this area.
