We couldn't find anything positive to say about this nursing home. It received a D in our short-term care rating. Our short-term care grade is often employed to gauge a nursing home's rehabilitation services. In order to have highly rated rehabilitation services, nursing homes generally must provide higher levels of highly skilled nursing services. These types of services include registered nurses, physical therapists, respiratory therapists and other highly skilled individuals. With a grade in this range for this category, we were not stunned to find this facility was below average in several staffing areas we looked at. The nursing home provided substantially fewer registered nurse and physical therapist hours per resident than most other nursing homes. Lastly, we assessed the percentage of patients who were able to return to the community from this facility. This definitely was not a strength for this facility. We found that just 3.6 percent of this facility's patients returned home. At most nursing homes, about half of their residents are able to return home so this is well below average.

This place didn't receive glowing government inspections in recent years. We gave it just a D for our inspections rating and that actually proved to be its second highest grade. Our inspection ratings factor in many items on the facility's recent inspection reports. If you see severe deficiencies on the reports, this is typically the worst sign. Severe deficiencies indicate a risk to patient safety. This nursing home had 6 deficiencies on its inspection report. The only positive thing we found is that none of its deficiencies were in the categories that suggest that they caused a risk to resident health or safety. Finally, we would like to point out that this facility received large government fines recently totaling more than $100,000. This often indicates that a bad performance spanned several years.

This facility ended up receiving rock bottom score in our nursing category. It received an F in this category. Nursing grades are largely tied to a facility's level of nurse staffing. This place averaged a meager 0.3 hours of nursing care per patient per day. Lastly, we also looked at some nursing quality measures in computing our nursing scores. This place performed well when it comes to avoiding major falls and pressure ulcers.

The final area we analyzed is long-term care. We gave it an F in this category, which is a poor grade. If you are looking for services other than rehabilitation, you should take a close look at each nursing home's long-term care grades. On top of assessing the amount of care provided by nurses aids and other staff, we analyzed the number of residents vaccinated against pneumonia. This facility administered the vaccine to 94.17476 percent of its residents, which is higher than the majority of nursing homes. Surprisingly, this nursing home was able to limit hospitalizations. With only 0.02 hospitalizations per one thousand long-term resident days, this facility has fewer hospitalizations than the majority of nursing homes. Sadly, a few of its other scores in the category were not as favorable as these.
