Facility Inspections
In addition to being a great overall grade, this facility also received A+ health inspections in recent years. Its inspections were virtually flawless. Inspection ratings weigh several factors included in a facility's inspection report. One key criteria we weigh heavily is the quantity and severity of deficiencies. Places with better grades in this area generally have few severe deficiencies. This facility was assessed 8 deficiencies on its inspection report, but we were relieved to see that none of the deficiencies were considered to be severe. This indicates that CMS didn't deem any of these deficiencies to be an imminent risk to patient safety or health. Finally, this facility had no substantiated complaints this year from residents, which is an excellent sign.
Long-term Care Quality
We also awarded this nursing home a grade of A for our long-term care score. When facilities receive this kind of grade in long-term care it is usually a good sign for resident care and indicates that the place is well-staffed with nurses aids. In addition to looking at the amount of care provided by nurses and other staff, we also looked at the percentage of residents vaccinated for pneumonia. This nursing home vaccinated 100 percent of its patients, which is much higher than the majority of nursing homes. Finally, this place was able to limit hospitalizations. It had only 0.02 hospitalizations per 1,000 long-term resident days, which is a remarkably low number.

Short-term Care Quality
The next category we graded was short-term care. In that area, we gave this facility a B-. Our short-term care grade is typically used to score a facility's performance with rehabilitation To provide quality rehabilitation services, nursing homes generally must offer higher levels of skilled nursing services. These types of services include registered nurses, physical therapists, respiratory therapists and other types of therapists. We weren't able to track down information about this nursing home's registered nurse or physical therapy staffing. Finally, we considered the number of residents who were able to return to the community from this facility. We found that 5.7 percent of this facility's residents returned home. At most facilities, closer to half of their residents are able to return home so this figure is below average.
Nurse Quality
The next area we looked at is nursing, which was a weak spot for this nursing home. We awarded this facility a grade of D for the area of nursing, rendering this the facility's weakest grade. We'd still recommend that you look at the bigger picture. There are many criteria included in this category. Many of these data points are tied to nurse staffing. This particular nursing home provided a meager 0.3 hours of nursing care per resident per day. This statistic is significantly below the national average. While this facility's nursing hours are below average, it surprised us in a few of the qualitative metrics in this category. In particular, we were pleased to see that its patients are sustaining fewer pressure ulcers than the national average.
