Facility Inspections
While this facility's overall grade was as bad as it gets, it actually really excelled in the category of inspections. In fact, we awarded it an A- for that category, which is one of our highest scores. These inspection grades take several factors into consideration, including deficiencies, substantiated complaints and federal fines. You can learn more about each of these issues by obtaining copies of nursing homes' inspection reports. This particular nursing home had just a single deficiency on its inspection report and it was not one deemed to pose a threat to patient health or safety. Even with this deficiency, this was still a relatively favorable inspection report.
Short-term Care Quality
Unfortunately, this nursing home only earned a D for its short-term care grade, which was actually one of its more respectable category scores. In determining these short-term care grades, we look at the nursing home's skilled nursing services, such as those performed by registered nurses, occupational therapists, physical therapists and other types of therapists. The goal is to devise a measure for sizing up the rehabilitation services of various facilities. Not surprisingly, we found that this nursing home provides far less physical therapist and registered nurse hours per resident than the average facility. Finally, we considered the percentage of patients that ultimately were able to return home from this nursing home. This nursing home didn't fare well here either. In fact, we found that just 4.7 percent of this facility's residents were able to return home. This figure was quite a bit off the national average.

Long-term Care Quality
Another unfortunate development was the F this facility was given in the area of long-term care. In a long-term care setting, the facility's primary goal is to maintain residents' quality of life and keep them safe. After looking at the amount of care provided by aids and other staff, we then analyzed the nursing home's vaccination records. This facility vaccinated 97.85276 percent of its patients against pneumonia, which is better than the average nursing home. To our surprise, this facility also fared well at keeping its patients out of the hospital. It had just 0 hospitalizations per one thousand long-term resident days. Sadly, a few of its other scores in this category were not as favorable as these.
Nurse Quality
The next area we assessed is nursing. It received an F in this category, which is a bottom of the barrel score. Our nursing grade considers a variety of factors. The most important one is the quantity of hours nurses spend with residents. This facility provided a meager 0.3 hours of nursing care per patient each day. This is an alarmingly low total. To go along with its subpar totals in the area of nursing hours per resident, this facility also did not perform as well in the quality-based measures we looked at in computing our nursing grades. We looked at the percent of residents sustaining pressure ulcers and major falls. This facility had more pressure ulcers and falls than the average facility.
