To go along with its strong overall grade, we awarded this nursing home a grade of A+ for our inspections rating. Arguably the most significant factor we consider in calculating our inspection grades is deficiencies. These deficiencies are found on a nursing home's inspection reports. Places with higher scores in this category typically dodged the more severe deficiencies involving things like patient abuse. Although this nursing home had a few deficiencies on its report, none were major deficiencies based on CMS' deficiency scale. Finally, this facility had no substantiated complaints this year from residents, which is an excellent sign.

We also want to draw your attention to the fact that this nursing home was given a first-rate grade of A- in our short-term care category. In crafting our short-term care ratings, we look at the facility's skilled nursing services, such as those performed by registered nurses, respiratory therapists, physical therapists and other types of therapists. Our purpose is to formulate a scale for sizing up the rehabilitation services of nursing homes. Unfortunately, it doesn't look like this facility submitted data regarding its registered nurse or physical therapy staffing. Lastly, we assessed the percentage of residents who were able to return to the community from this facility. This proved to be a real strength for this facility. We found that it outperformed the vast majority of nursing homes in the country with 59.3 percent of its patients returning home. This is a significantly better rate than most nursing homes.

Building on its remarkable assessment, this nursing home also was given a superb long-term care score, with a score of A-. Long-term care grades of this caliber generally require both around the clock care from nurses and aids, as well as quality routine medical care. In addition to considering the amount of care provided by aids and other staff, we looked at the percentage of residents vaccinated against pneumonia. This nursing home vaccinated 100 percent of its residents, which is an impressive figure. Pneumonia tragically can be a dangerous health condition for nursing home residents so we prefer when a nursing home doesn't take any chances. Lastly, this nursing home was able to limit hospitalizations. It had less than one hospitalization per 1,000 long-term resident days, which is a remarkably low number.

The last category we looked at is nursing. In that category, we gave this nursing home an acceptable grade of B-. This was this nursing home's worst category. There are a number of criteria within this area. Many of these data points are tied to the quantity of nurse staffing. This nursing home averages just 0 hours of nursing care per resident per day. This is a very low figure. Finally, we also factored some quality-based metrics into our nursing ratings. This nursing home was subpar in two of the major areas we look at, with below average scores for minimizing its patients' pressure ulcers and major falls. These areas are usually decent indicators of the quality of nursing care available.
