This nursing home received a solid overall grade as described above, but we really wanted to emphasize its excellent inspection reports in recent years. We awarded them an impressive grade of A in this category. We weigh several aspects of a nursing home's inspection report in determining these inspection scores. One critical factor is deficiencies. It should be noted that the severity of the deficiencies is arguably more meaningful than the quantity of deficiencies, as some are relatively minor. Although this facility had some deficiencies on its inspection report, none of them were serious based on CMS' scale. A couple minor deficiencies shouldn't lead you to cross a nursing home off your list.

This nursing home also was given a favorable long-term care score. Indeed, we gave it a B in this category, which happens to be one of our better. Facilities that do well in this category typically provide residents with closer supervision and stay on top of routine healthcare services. In addition to looking at the quantity of care provided by nurses aids and other staff, we looked at the number of residents vaccinated against pneumonia. This facility vaccinated 95.43379 percent of its residents. Vaccines are vital to keeping patients out of the hospital. Lastly, this facility was able to limit hospitalizations. In fact, it had less than one hospitalization per one thousand long-term resident days, which is a remarkably low number.

Another concern was the F this nursing home was given in the area of short-term care. In the area of short-term care, we endeavor to qualify measures of a nursing home's rehabilitation services. We look at the nursing home's skilled nursing services, including the ones performed by registered nurses and various therapists. Considering its grade in this category, we weren't surprised to learn that this place is well below average in terms of its number of physical therapy and registered nurse hours offered to its residents based on the measures we looked at. Lastly, we looked at the percentage of residents who were able to return to the community from this facility. We found that just 0 percent of this facility's patients returned home.

The last category we rated is nursing, which proved to be this nursing home's worst category. It received a lowly F in this category, which is obviously a major concern. Nursing grades are based in large part on quantity of nursing care available. This facility provided only 0.2 hours of nursing care per resident daily. This is not a very impressive figure. To go along with its below average total nursing hours per resident, this nursing home had bad marks in some of the quality-based measures we looked at in determining our nursing grades. We looked at the percent of patients experiencing falls leading to major injury. This nursing home was at more than 150 percent of the national average in this statistic. This is alarming in light of the fact that so many falls could be avoided with better nursing care. This ended up sealing the nursing home's fate of receiving a poor nursing grade.
