Although we gave this facility a terrible overall grade, we awarded it a C for our inspections rating. In fact, the facility received very impressive inspection report this year. These inspection ratings weigh several factors, including deficiencies and federal fines. This facility was hit with 8 deficiencies on its inspection report, but fortunately none were in categories J through L. This tells you the government inspectors didn't deem any of the deficiencies to put resident safety or health in imminent harm or jeopardy. In addition to having severe deficiencies, this nursing home was cited by CMS for possible abuse or neglect. This is generally a really bad sign. We would never recommend this facility.

Sadly, this nursing home only earned a D for its short-term care rating, which ended up being one of its higher category scores. With our short-term care assessment, we attempt to create a fair barometer for rehabilitation services. In doing so, we assess a nursing home's level of skilled nursing services, including both registered nurses and physical therapy, as well as respiratory therapy. It appears that this facility was subpar in multiple staffing areas we looked at. The facility offered fewer registered nurse and physical therapist hours per patient than most other facilities. Lastly, we looked at the number of residents who were able to return to the community from this nursing home. We were surprised to find that it performed better in this area, with 52.6 percent of its patients returning home. This was just about this nursing home's sole highlight in this category.

In addition, we gave this facility an F in the area of nursing care. Nursing ratings are mostly based on a nursing home's nurse staffing. This nursing home provided only 3.2 hours of nursing care per resident per day. This is an alarmingly low total. To pair with its low totals in the area of nursing hours per patient, this facility also did not fare as well in the quality-based measures we looked at in computing our nursing scores. We looked at the percentage of patients sustaining pressure ulcers and major falls. This place had more pressure ulcers and falls than the average facility. This may be a bad sign when you consider that many bed sores and falls are believed to be preventable with better nursing care. Unfortunately, these statistics pulled down this facility's nursing grade quite a bit.

The final area we looked at is long-term care. This facility received a poor grade of F in this area. For long-term care residents, the primary objective is to maintain patients' quality of life and keep them safe. One of the data points we considered on top of nursing hours is vaccines. Thankfully, this nursing home provided the pneumonia vaccination to 96.84211 percent of its patients. To our surprise, this nursing home was able to limit hospitalizations. With just 1.41 hospitalizations per 1,000 long-term resident days, this facility had fewer hospitalizations than most nursing homes. Unfortunately, a few of its other scores in this category were not as favorable as these.
