Facility Inspections
This nursing home received a solid overall grade as described above, but we really wanted to emphasize its nearly flawless inspection reports in recent years. We gave them an impressive grade of A in this category. Inspection grades account for several factors included in a nursing home's inspection report. One of the most important criteria we consider is the quantity and severity of deficiencies. Nursing homes with better grades in this category generally have few of these severe deficiencies. Although this nursing home had some deficiencies on its inspection report, none of them were major deficiencies based on CMS' deficiency scale. A couple of minor deficiencies should not lead you to cross a nursing home off your list.
Short-term Care Quality
This nursing home's short-term care score appears to be one of its better scores. The facility received a grade of B- in this area. In determining our short-term care scores, we look at a nursing home's levels of skilled nursing, which includes registered nurses, physical therapists speech therapists and other highly trained professionals. This area is considered to be a reliable assessment of the facility's rehabilitation. Unfortunately, we found that this place provided fewer registered nurse and physical therapist hours per resident than most other facilities. The last metric we considered in this category is the percentage of residents who eventually returned home from the facility. We found that just 4.9 percent of this nursing home's patients returned home.

Nurse Quality
This facility's next best category was its nursing grade, where it received a grade of B-. The nursing grade is based on quite a few components, but the main consideration is the level of nurse hours per patient per week. This nursing home provides 0.3 hours of nursing care per patient on a daily basis. This is a much lower figure than we are used to seeing. Lastly, nursing scores also consider quality-based measures, such as avoiding major falls. Despite not having the highest total nursing hours, this nursing home performed admirably in the area of preventing major falls. This is usually a reliable indicator that a place has quality controls in place. Some falls can be avoided if a nursing home supplies enough nurses to assist its residents.
Long-term Care Quality
The final category we assessed was long-term care, which was a weak spot for this nursing home. We gave this facility a grade of D in long-term care. For prospective patients looking for a permanent place to live rather than rehabilitation, long-term care is a very important category. In addition to looking at the volume of care provided by nurses aids and other staff, we analyzed the percentage of residents vaccinated against pneumonia. This facility provided the vaccine to 95.9854 percent of its residents, which is higher than the vast majority of nursing homes. To our surprise, this facility was able to limit hospitalizations. With just 0.01 hospitalizations per one thousand long-term resident days, this facility has fewer hospitalizations than the majority of nursing homes. Unfortunately, some of its other scores in this category weren't as favorable as these.
