Although we gave this facility a terrible overall grade, we awarded it a grade of C for our inspections rating. In fact, the facility received very impressive inspection report this year. We would argue that one of the most important factors in evaluating an inspection report is deficiencies. In this regard, we especially look at the severity of those deficiencies. This facility was assessed 6 deficiencies on its government inspection report, but none were in categories J through L. This indicates that the government inspectors didn't deem any of the deficiencies to put resident safety or health in imminent harm or jeopardy. Unfortunately, we also need to draw your attention to the fact that this facility received some government fines in recent years.
Short-term Care Quality
Sadly, this facility's second best score was short-term care. Nevertheless, with a D in this area, it still fared somewhat poorly. In the category of short-term care, we strive to evaluate measures of a nursing home's rehabilitation services. We look at a facility's skilled nursing services, such as those performed by registered nurses and physical, occupational and other therapists. Not surprisingly, we learned that this nursing home provides significantly less registered nurse and physical therapist hours per patient than the average nursing home. Finally, we assessed the percentage of residents who were able to return to the community from this facility. We found that just 4.6 percent of this facility's patients returned home.

We gave this facility a grade of just D for our nursing rating. When calculating our nursing grades, we look at both the number of hours nurses spend with residents and the training levels of those nurses.
