Facility Inspections
The primary reason this turned out to be a decent nursing home is that it earned an elite inspection score. Its inspection grade was far better than its overall grade. In our inspections category, we gave this facility an A. Inspection grades weigh several factors found on a nursing home's inspection reports. One key criteria we weigh heavily is the quantity and severity of deficiencies. Nursing homes with better scores in this area typically have very few severe deficiencies. While this place had some deficiencies on its inspection report, none were serious based on CMS' scale. A couple minor deficiencies shouldn't stop you from considering a nursing home.
Long-term Care Quality
This facilities next best area was long-term care. We gave it a grade of B- in this area. This is essentially a middle of the road score in this category. In calculating our long-term care grades, we analyze the personal care received by the nursing home's patients. In addition to looking at the quantity of care provided by nurses and other staff, we looked at the number of residents vaccinated for pneumonia. Fortunately, this facility provided the vaccine to 100 percent of its residents, which is a very solid statistic. Finally, this place was able to limit hospitalizations. With just 1.29 hospitalizations per one thousand long-term resident days, this nursing home had fewer hospitalizations than most nursing homes.

Short-term Care Quality
Sadly, this nursing home only received a D for its short-term care grade, which is not one of our better scores. In calculating our short-term care grades, we size up a facility's levels of skilled nursing services, which includes registered nurses, physical therapists speech therapists and other highly skilled individuals. This rating is more often than not a solid assessment of the facility's ability to rehabilitate patients. With its weak grade in this category, we weren't surprised to learn this facility offers substantially fewer physical therapist and registered nurse hours per patient than most other nursing homes. Finally, we considered the percentage of residents that ultimately returned home from this nursing home. It fared better in this area, with 52 percent of its residents returning home.
Nurse Quality
Switching gears to the next category, we awarded this nursing home a D for nursing. This is the facility's weakest category rating. In computing a nursing home's nursing score, we weigh the number of hours nurses spend with patients as well as the level of licensure of the nurses. This particular nursing home provided 3.5 hours of nursing care per resident per day, which is below average. To pair with its low total nursing hours per patient, this nursing home had really bad marks in some of the quality-based metrics we looked at in computing our nursing grades. For starters, we looked at the percent of patients sustaining falls which led to major injury. This nursing home was at more than 1.5 times the national average in this statistic.
