Facility Inspections
Although this facility's overall grade was as bad as it gets, it performed pretty well in the category of inspections. In fact, we awarded it a B for that area, which is one of our better scores. Arguably the most critical factor we look at in determining our inspection ratings is deficiencies. These deficiencies can be found on a facility's inspection reports. It is generally best to avoid nursing homes that have too many severe deficiencies. While this nursing home had a few deficiencies on its report, none of them were serious based on CMS' deficiency scale. A few minor deficiencies aren't the end of the world.
Short-term Care Quality
Unfortunately, this nursing home only earned a grade of D for its short-term care score, which happens to be one of its higher category grades. In calculating these short-term care grades, we assess the facility's skilled nursing services, including those performed by registered nurses, occupational therapists, physical therapists and other types of therapists. Our purpose is to create a scale for sizing up the rehabilitation services of nursing homes. Not surprisingly, we found that this nursing home provides far less physical therapist and registered nurse hours per resident than the average facility. Finally, we assessed the number of residents that returned home from this nursing home. It performed better in this area, with 57.3 percent of its residents returning home.

Long-term Care Quality
This nursing home's second worse category is long-term care, where it earned a grade of just D in that area. This is a well below average score. Nursing homes that do not fare well in long-term care typically aren't as well-staffed and are lagging in a few of the areas of routine healthcare services we assessed. Once we finished looking at the volume of care provided by nurses, we then looked at the nursing home's vaccination statistics. This nursing home vaccinated 93.75 percent of its patients for pneumonia. Surprisingly, this nursing home was actually able to limit hospitalizations. With just 1.02 hospitalizations per 1,000 long-term resident days, this facility has fewer hospitalizations than most nursing homes. This is its best feature in this category.
Nurse Quality
The next category we looked at is nursing. It received an F for this area, which is a rock bottom score. In determining a nursing home's nursing rating, we consider the amount of time nurses are seeing patients as well as the level of licensure of those nurses. This nursing home offered a meager 3 hours of nursing care per resident per day. This is not a very impressive figure. On top of receiving below average marks for nursing hours, this facility was less impressive in some of the quality-based metrics we looked at in computing our nursing ratings. We looked at the percent of patients suffering pressure ulcers and found that this facility was at roughly 150 percent of the national average in this statistic. This is likely a bad sign when you consider that many bed sores are preventable with better nursing care. Unfortunately, this statistic pulled down this facility's nursing rating significantly.
