While we graded this facility incredibly poorly overall, it received a fairly respectable nursing grade this year. We gave it a grade of C in that category. When calculating a facility's nursing grade, we consider the amount of time nurses are caring for residents as well as the skill levels of those nurses. Based on the CMS data, this nursing home offers 3.5 hours of nursing care per resident per day. Finally, we also considered quality-based metrics in computing our nursing ratings. This facility was above average in two of the major datapoints we assessed, including minimizing its residents' major falls and pressure ulcers.

We gave this facility just a D for our long-term care rating and that actually ended up being its next best category. Facilities that don't fare well in long-term care typically are not as well-staffed and are lagging in a few of the areas of routine medical care we assessed. In addition to looking at the volume of care provided by nurses aids and other staff, we also looked at the percentage of residents vaccinated for pneumonia. Candidly, we were a bit worried that this nursing home gave the vaccine to just 84.48276 percent of its residents. Finally, we were stunned by the facility's number of hospitalizations. Here we found that this place fared very poorly with 4.35 hospitalizations per 1,000 long-term resident days. Unfortunately, this figure is more than double the national average.

Historical

We awarded this nursing home a grade of just D for our short-term care score. This is not an impressive grade. Our short-term care ratings are vital for individuals seeking rehabilitation. Rehabilitation generally utilizes higher levels of highly-skilled nursing services. This means a wide spectrum of services, spanning from registered nurses to physical and respiratory therapists, as well as other types of therapy. Unfortunately, we found that this nursing home provided fewer physical therapist and registered nurse hours per resident than most other facilities. The final metric we looked at in this category is the percentage of residents who were able to leave the nursing home and return to the community. We discovered that just 36.7 percent of this facility's residents were able to return home, which is well below average. The combination of these poor statistics doomed this nursing home's short-term care grade.

In our final area of inspections, this nursing home received an F in this area. Since this information is collected by government inspectors, we heavily weight negative feedback in this category. We would advise paying special attention to any deficiencies on a facility's inspection reports when it receives this poor of an inspection grade. We suggest you pay close attention to the level of severity of those deficiencies. This particular facility was hit with one category G through L deficiency, which rank among the more severe categories of deficiencies. These categories indicate that the deficiencies identified by inspectors had the potential to cause actual harm to residents. Finally, we want to point out that this nursing home received large government fines recently amounting to more than $100,000. This typically means that a nursing home's bad performance has been an issue for several years.
