While we graded this nursing home very poorly overall, it received a somewhat respectable nursing rating this year. We gave it a grade of B- in that area. The nursing category analyzes several datapoints, most of which are based on nursing hours. Based on the CMS data, this nursing home offers 3.4 hours of nursing care per resident per day. Finally, we also assessed some nursing quality-based metrics in determining our nursing ratings. This nursing home performed well in terms of avoiding major falls and pressure ulcers.

We were also pleasantly surprised to learn that this facility received a reasonably solid long-term care grade this year. We gave it a grade of C in this area. For residents seeking a permanent place to live rather than skilled nursing, long-term care grades are a key measure. One of the datapoints we considered on top of nurse's aid hours was vaccinations. Thankfully, this nursing home administered the pneumonia vaccine to 100 percent of its residents. This facility keeps its patients out of the hospital. While it had 3.32 hospitalizations per one thousand long-term resident days, its risk adjusted score was better than the majority of nursing homes since it had more complicated patients.

An F in this area is typically a signal that we found red flags on the facility's inspection reports. When a nursing home has this bad of an inspection grade, you should expect some severe deficiencies on its inspection report. Severe deficiencies indicate a threat to resident safety. This facility had 9 deficiencies on its inspection report. The only positive thing we found is that none of its deficiencies were in the categories that indicate they created a risk to resident health or safety. The next area we rated is short-term care, in which this facility was given a very poor score in this area as well. It received an abysmal F for this category, which is obviously a major concern. Our short-term care grade is commonly employed to grade a facility's performance with rehabilitation. To have highly rated rehabilitation services, facilities generally must have better levels of highly skilled nursing services. These types of services include registered nurses, physical therapists, respiratory therapists and other highly skilled individuals. With a grade in this range in this area, we weren't stunned to learn this place was well below average in two of the key staffing areas we focus on. The nursing home provided significantly fewer physical therapist and registered nurse hours per resident than most other facilities. Finally, we looked at the percentage of patients who were able to return to the community from this facility. This wasn't a strength for this facility. We found that just 32 percent of this nursing home's patients returned home. At most facilities, about half of their patients are able to return home so this is well below average.
