Facility Inspections
B-
Historical
While we graded this facility incredibly poorly overall, it received a fairly respectable inspection report this year. As a result, it received a decent grade in that category with a score of B-. Our inspection grades weigh several factors found on a nursing home's inspection report. One of the most important criteria we rely on is the number and severity of deficiencies. This facility was hit with 3 deficiencies on its inspection report, but none of the deficiencies were considered to be severe. This means that the government inspectors did not consider any of these deficiencies an immediate risk to patient health or safety. Lastly, this nursing home was cited by CMS for possible abuse or neglect. This is generally a really bad sign. We would never recommend this facility.
Long-term Care Quality
D
Historical
Unfortunately, this nursing home only earned a grade of D for its long-term care grade, which turns out to be one of its better category grades. In a long-term care setting, the facility's primary goal is to keep patients healthy and safe. This is different than skilled nursing or short-term care where the goal is to rehabilitate residents. On top of considering the quantity of care provided by nurses aids and other staff, we looked at the number of residents vaccinated for pneumonia. This nursing home provided the vaccine to 97.17742 percent of its patients. Lastly, we looked at its number of hospitalizations. We found that this place fared much worse with 5.32 hospitalizations per 1,000 long-term resident days. Sadly, this statistic is more than double the national average.

Nurse Quality
D
Historical
We gave this facility a grade of just D for our nursing rating. Our nursing grade is primarily based on a nursing home's level of nurse staffing. With a meager 1.7 hours of nursing care per resident per day, this facility's nurse staffing levels were well below the national average. To pair with its below average totals in the area of nursing hours per resident, this facility had bad scores in many of the quality-based metrics we looked at in calculating our nursing scores. We looked at the percentage of residents having falls which led to major injury. This nursing home was at more than 150 percent of the national average in this statistic.
Short-term Care Quality
F
Historical
The last category we assessed was short-term care. Sadly, it received a lowly F in this category, which is a bottom of the barrel grade. In our short-term care score, we seek to craft a fair measure for rehabilitation services. In this process, we assess a facility's scope of skilled nursing services, including both registered nurses and physical therapy, as well as respiratory therapy. With its score in this category, we were not surprised to learn that this place is well below average in terms of the quantity of registered nurse and physical therapy hours offered to its residents based on the metrics we looked at. Finally, we looked at the number of patients that ultimately returned home from this nursing home. This nursing home didn't fare well here either. In fact, we found that just 19.3 percent of this facility's residents were able to return home. This figure was well off the national average.
