While this nursing home's overall score was not a highlight, it actually excelled in the category of inspections. In fact, we awarded it an A+ for that category, which is one of our highest scores. We take several aspects of a facility's inspection report into consideration in calculating these inspection grades. One critical factor is health deficiencies. It should be noted that the severity of these deficiencies is usually more important than the quantity of deficiencies, as some are quite insignificant. While this nursing home had some minor dings on its inspection report, it had zero severe deficiencies. Severe deficiencies are those labeled as categories G through L. This means that the government inspectors did not consider any of the deficiencies on this facility's report to pose an imminent threat to patient safety or health. A few minor deficiencies aren't the end of the world.
We also want to point out that this facility received a decent grade in our nursing category. We awarded it a grade of C in the area. We scrutinized the licensure of nurses at the facility, in addition to the number of hours the nurses worked with patients, in calculating our score in this category. This particular facility provided just 3.3 hours of nursing care per patient daily. This figure is significantly below average. Although this nursing home didn't really impress in terms of the quantity of nursing care available, it performed admirably in a few of our quality-based metrics. In fact, it was above average in two of the areas we assess, with good scores for minimizing its residents' major falls and pressure ulcers. These statistics are reliable indicators of the quality of nursing care provided since many bed sores and falls can be avoided.

Long-term Care Quality
We gave this nursing home a grade of just D for our long-term care rating. This is not one of our better scores. In a long-term care environment, the nursing home's primary goal is to keep patients healthy and safe. This is different than skilled nursing or short-term care where the goal is to rehabilitate patients. After we finished looking at the quantity of care provided by nurses, we then looked at the nursing home's vaccination data. We were optimistic to find that this nursing home vaccinates 100 percent of its patients for pneumonia. Surprisingly, this facility also fared well at keeping its patients out of the hospital. It had only 1.49 hospitalizations per 1,000 long-term resident days. Sadly, a few of its other scores in this category were not as favorable as these.
Short-term Care Quality
The next area we assessed was short-term care. This nursing home was given a rock bottom grade of F in this area. In crafting these short-term care scores, we look at the facility's skilled nursing services, including those performed by registered nurses, speech therapists, physical therapists and other types of skilled professionals. The purpose is to devise a tool for sizing up the rehabilitation services of different nursing homes. With its grade in this category, we weren't surprised to discover that this place is well below average in terms of the quantity of registered nurse and physical therapy hours offered to its patients based on the figures we assessed. Honestly, this was about what we expected to find. The last item we assessed in this area is the number of patients who were able to leave the nursing home and return to the community. We discovered that just 20.3 percent of this facility's residents were able to return home, which is well below average. The combination of these subpar statistics doomed this nursing home's short-term care grade.
