Nurse Quality
B-
Historical
Although we graded this facility extremely poorly overall, it received a fairly respectable nursing rating this year. We gave it a B- in this area. In calculating a facility's nursing grade, we consider the amount of time nurses spend with residents as well as the skill levels of the nurses. Based on the data, it looks like this facility provides an impressive 4.1 hours per patient each day. This is more nursing care than most places offer. In computing our grades, we apply more weight to hours performed by more highly trained nurses such as registered or licensed nurses. Lastly, our nursing grades also factor in some quality-focused statistics. Specifically, we look at the percent of residents who sustained pressure ulcers and major falls. We look at these statistics as good measures of the quality of nursing care, since better care tends to reduce these problems.
Short-term Care Quality
B-
Historical
Surprisingly, we gave this facility a B- for its short-term care score, which really isn't too bad of a score. In our short-term care rating, we endeavor to create a valuable gauge for rehabilitation services. In doing so, we analyze the facility's scope of skilled nursing services, including both registered nurses and physical therapy, as well as respiratory therapy. This nursing home is respectable both in terms of its quantity of physical therapy and registered nurse hours provided to its residents. The last measure we looked at in this category is the percentage of residents who were able to leave the nursing home and return to the community. It was respectable in this metric with 46 percent of its patients returning home.

Long-term Care Quality
D
Historical
This facility's second least favorable category is long-term care, where it earned just a D in that area. This is a relatively poor score. For residents looking for a permanent residence rather than skilled nursing, long-term care is an important category. Once we finished looking at the quantity of nursing care, we then analyzed the facility's vaccination record. We were pleased to find that this nursing home vaccinates 98.314606 percent of its patients against pneumonia. Surprisingly, this facility was able to limit hospitalizations. With just 1.55 hospitalizations per 1,000 long-term resident days, this nursing home has fewer hospitalizations than many nursing homes. Sadly, a few of its other scores in the category were not as favorable as these.
Facility Inspections
F
Historical
In our last category of inspections, this facility received a grade that's as bad as it gets. We weight this grade more heavily than our other categories. With places that received this poor of an inspection score, we would scrutinize the severe deficiencies on its inspection report. This nursing home was assessed a category G through L deficiency, which rank among the more serious deficiencies. These categories indicate that the deficiencies uncovered by CMS had the potential to pose actual harm to patients. In addition to having severe deficiencies, this nursing home was cited by CMS for possible abuse or neglect. This is generally a really bad sign. We would never recommend this facility.
