While we graded this facility incredibly poorly overall, it received a fairly respectable inspection report this year. As a result, it received a decent grade in that category with a score of B-. We take several aspects of a facility's inspection report into consideration in calculating our inspection scores. One key factor is health deficiencies. It should be noted that the severity of deficiencies is usually more meaningful than the quantity of deficiencies, as some end up being quite insignificant. Although this place had a few deficiencies on its report, none were severe based on CMS' deficiency scale. Lastly, this facility was flagged by CMS for possible abuse or neglect. This facility's next most respectable category was short-term care, but it still received a grade of just D in that area. This turns out to be a relatively poor grade. In the area of short-term care, we try to assess indicators of a nursing home's rehabilitation. We assess the facility's skilled nursing services, including those performed by registered nurses and various therapists. Not surprisingly based on its grade in this category, this facility was subpar in one of the two key staffing areas we looked at. The facility provided far fewer registered nurse hours per patient than most other facilities. Finally, we assessed the percentage of patients who were able to return to the community from this facility. This definitely was not a strength for this facility. We found that just 39.6 percent of this facility's patients returned home. At most facilities, around half of their patients are able to return home so this is well below average.

Among this nursing home's several poor grades was its F in the area of nursing care. Our nursing grade considers a host of factors. The most important variable is the number of hours nurses spent with residents. This place offered only 2.9 hours of nursing care per resident per day. This is well below average. To pair with its subpar totals in the area of nursing hours per patient, this place also did not fare as well in the quality-based measures we looked at in determining our nursing grades. We looked at the percentage of residents sustaining major falls and pressure ulcers. This facility had more falls and pressure ulcers than the average nursing home. This may be a bad sign when you consider that many falls and bed sores are believed to be preventable with better nursing care. These metrics pulled down this facility's nursing rating significantly.

The final area we graded was long-term care. We gave it an F for this category, which is a very poor grade. When facilities receive a score in this range in long-term care it typically means the nursing home did not perform well in our quality measures relating to resident care. On top of assessing the quantity of care provided by nurses aids and other staff, we also looked at the percentage of residents vaccinated against pneumonia. This nursing home administered the vaccine to 86.585365 percent of its residents. This is many points below what we were hoping for. To our surprise, this place was actually able to limit hospitalizations. With only 1.69 hospitalizations per one thousand long-term resident days, this facility had less hospitalizations than the average nursing home. This is its best score in this category.
