Facility Inspections
Although we gave this facility a weak overall grade, we gave it a somewhat respectable score in the category of inspections, where it received a B-. This was quite a bit better than its other category grades. We take several aspects of a facility's inspection report into consideration in calculating these inspection grades. One key factor is deficiencies. It should be noted that the severity of deficiencies is usually more important than the number of deficiencies, as some of these can be quite insignificant. While this place had some minor dings on its inspection report, it had zero severe deficiencies. Severe deficiencies are those found in categories G, H, I, J, K and L. This tells you that the government inspectors didn't consider any of the deficiencies on this facility's report to pose an imminent threat to patient health or safety. A couple minor deficiencies aren't necessarily the end of the world.
Long-term Care Quality
This facility's second highest category was long-term care. In that area, we gave this nursing home a grade of B-. In calculating our long-term care grades, we size up the assistance with daily living offered to a facility's patients. On top of assessing the volume of care provided by nurses and other staff, we analyzed the percentage of residents vaccinated against pneumonia. This nursing home administered the vaccine to 100 percent of its patients, which is better than the majority of nursing homes. This nursing home was able to keep its patients out of the hospital. In fact, it had less than one hospitalization per 1,000 long-term resident days, which is a remarkably low number.

Short-term Care Quality
Unfortunately, this nursing home's next highest score was short-term care. Nevertheless, with a D in this area, it still fared somewhat badly. Our short-term care grade is often used to score a facility's rehabilitation services To provide quality rehabilitation services, nursing homes generally need to feature better levels of highly skilled nursing. These types of services include registered nurses, physical therapists, respiratory therapists and other licensed professionals. Considering its weak score in this category, we were not stunned to learn that this nursing home offers significantly fewer physical therapist and registered nurse hours per patient than most other facilities. Lastly, we assessed the number of patients that ultimately returned home from this nursing home. This facility didn't fare well here either. In fact, we found that just 0 percent of this facility's patients were able to return home.
Nurse Quality
The final category we assessed is nursing, proved to be this nursing home's least impressive category. We gave this facility a D in that area. Our nursing score analyzes the nursing home's nurse staffing levels. We factor in both the levels of training of the nurses as well as the quantity of hours spent with residents. With only 0.3 hours of nursing care per resident per day, this facility's nurse staffing levels were far below average. Lastly, we also looked at a few quality-based metrics in calculating our nursing grades. This nursing home was above average in two of the areas we considered, with good marks for avoiding major falls and pressure ulcers.
