Registered Staff Hours
Short-term Care Quality
While we graded this nursing home incredibly poorly overall, it received a fairly respectable short-term care grade this year. We gave it a C in that area, which was the closest thing to a bright spot we could find for this place. Our short-term care grade is often used to grade a facility's rehabilitation services. To provide highly scored rehabilitation services, nursing homes generally need to offer better levels of skilled nursing. These types of services include registered nurses, physical therapists, occupational therapists and other types of therapists. Unfortunately, we found that this nursing home provided fewer registered nurse and physical therapist hours per resident than most other facilities. The key is determining what bearing this had on quality of care. The last item we assessed in this area is the number of patients who were able to leave the nursing home and return to the community. We discovered that just 5.4 percent of this facility's patients were able to return home, which is below average.
Facility Inspections
This nursing home really didn't perform horribly in the category of inspections. In fact, we gave it a C for that category. Other categories turned out to be the source of this facility's rock bottom overall grade. Our inspection ratings are tied to items located in the a nursing home's recent inspection reports. Deficiencies are a key item to look for on these reports. Severe deficiencies indicate a threat to resident safety. This nursing home was assessed 4 deficiencies on its inspection report, but none were in any of the categories labeled G through L. This means that CMS did not consider any of the deficiencies to present an imminent danger to patient safety or health. Unfortunately, we also need to draw your attention to the fact that this facility received significant government fines in recent years.

Nurse Quality
Turning to the area of nursing care, this facility received a grade of just D in this category. When determining a facility's nursing score, we assess the number of hours nurses are with patients and the levels of training of those nurses. This place averaged only 0.4 hours of nursing care per resident each day. This is a very low total compared to most nursing homes. Lastly, we also assessed some nursing quality measures in calculating our nursing grades. This place fared well in the area of minimizing its residents' pressure ulcers and major falls. Many consider these areas to be reliable indicators of the quality of nursing care.
Long-term Care Quality
The last area we rated is long-term care. This nursing home was given a very poor grade of F in this category. Nursing homes that receive this type of grade in this category may not provide the kind of consistent around the clock care that some other nursing homes offer. After considering the amount of care provided by nurses and other staff, we next considered the facility's vaccination statistics. Frankly, we were a bit alarmed this nursing home vaccinated 26.848251 percent of its residents against pneumonia, which is significantly below what we were hoping for. To our surprise, this nursing home actually fared well at keeping its patients out of the hospital. It had only 0.01 hospitalizations per 1,000 long-term resident days. This was its best feature in this category.
