Facility Inspections
Although we were not high on this nursing home overall, we awarded it a grade of B+ for our inspections rating. This grade is much more impressive than the place's overall grade. Our inspection ratings are tied to many datapoints located in the a nursing home's recent inspection reports. Deficiencies are an important item to look for on these inspections. Most importantly, you should avoid nursing homes with severe deficiencies linked to endangerment of patients. Although this facility had a few deficiencies on its inspection report, none of them were severe based on CMS' scale. A few minor deficiencies aren't the end of the world.
Long-term Care Quality
We also gave this nursing home a respectable score in the category of long-term care, with a C. Long-term care grades focus on a facility's personal care instead of the skilled nursing or medical services that are critical to a rehabilitation facility. After we finished assessing the amount of care provided by nurses, we then analyzed the nursing home's vaccination data. We were pleased to learn that this nursing home vaccinates 100 percent of its residents for pneumonia. Lastly, this place was also able to limit hospitalizations. Indeed, it had only 0.03 hospitalizations per 1,000 long-term resident days, which is a remarkably low figure.

Nurse Quality
This nursing home actually received a decent grade in the area of nursing as well. We awarded it a grade of C for that category, which is not a bad score. Our nursing score consists of a handful of components, but the main one is the number of nurse hours spent with patients. This place provided just 0.4 hours of nursing care per patient daily. This is a relatively low total compared to most nursing homes. Despite having low quantities of nursing care, this facility actually excelled in some of the quality measures we assessed. In fact, it performed well when it comes to avoiding pressure ulcers and major falls.
Short-term Care Quality
The final category we graded was short-term care, in which this facility received a rock bottom score in this category as well. Sadly, it received an abysmal F in this area, which is obviously very concerning. In computing our short-term care ratings, we scrutinize the nursing home's levels of highly skilled nursing services, which includes registered nurses, physical therapists respiratory therapists and other highly skilled individuals. This score is often a fair measure of the facility's rehabilitation. Based on its abysmal grade in this area, we weren't surprised to learn that this place offers substantially fewer physical therapist and registered nurse hours per patient than most facilities. Finally, we considered the number of patients who were able to return to the community from this facility. This definitely was not a strength for this facility. We found that just 3.8 percent of this facility's patients returned home. At most nursing homes, around half of their residents are able to return home so this is well below average.
