Facility Inspections
We really could not find anything favorable to say about this facility. It received a D in the area of inspections. Sadly, this turned out to be its best grade. Inspection ratings account for several factors found on a facility's inspection report. One of the most important criteria we look at is the quantity and severity of deficiencies. This particular nursing home had 5 deficiencies by CMS. The only positive thing we can say is that none of the deficiencies were in the categories that indicate that they caused a risk to resident safety or health. Lastly, we need to point out that this place received large government fines amounting to more than $100,000. This generally means that a poor track record has been an issue for multiple years.
Long-term Care Quality
In addition, we gave this facility a rock-bottom F in the category of long-term care. Facilities that receive this kind of score in this category may not provide the type of consistent around the clock care that some other facilities provide. On top of assessing the quantity of care provided by nurses aids and other staff, we also looked at the percentage of residents vaccinated for pneumonia. This facility provided the vaccine to a relatively low 68.888885 percent of its patients, which is slightly concerning. Surprisingly, this place actually fared well at keeping its patients out of the hospital. Indeed, it had just 0.03 hospitalizations per 1,000 long-term resident days. This is its best feature in this category.

Short-term Care Quality
In our next category, we gave this facility a rock-bottom F in the category of short-term care. Short-term care ratings are based in part on a nursing home's quantity of highly-skilled nursing services. This includes a wide range of services, spanning from registered nurses to physical and respiratory therapists, as well as other types of therapy. Considering its abysmal grade in this area, we weren't surprised to discover that this nursing home provided substantially fewer physical therapist and registered nurse hours per patient than most other facilities. Finally, we assessed the number of patients who ultimately were able to return home from this facility. This facility didn't fare well here either. In fact, we found that just 0 percent of this facility's residents were able to return home. This figure was quite a bit off the national average.
Nurse Quality
The next area we looked at was nursing, in which this facility received a very poor score in this area also. Sadly, it received a lowly F in this area, which is obviously somewhat alarming. Our nursing grade weighs quite a few factors, many of which are associated with levels of nurse staffing. This facility provided just 0.1 hours of nursing care per resident per day. This is a very low total compared to most nursing homes. Finally, we also assessed some nursing quality-based metrics in determining our nursing ratings. This place performed well in the area of minimizing its residents' pressure ulcers and major falls.
