Facility Inspections
While we gave this facility a terrible overall score, we awarded it a grade of B for our inspections rating. This is the result of the facility receiving a quality government inspection report this year. Inspection ratings take several factors into consideration, including deficiencies, substantiated complaints and federal fines. You can learn more about each of these issues by reviewing copies of nursing homes' inspection reports. Fortunately, although this facility had a few minor dings on its report, it had no severe deficiencies. Severe deficiencies are ones labeled as categories G through L. This means that CMS didn't deem any of the deficiencies on this facility's report to pose an imminent threat to patient health or safety. A few minor deficiencies are not necessarily the end of the world.
Short-term Care Quality
Turning to the area of short-term care, this nursing home was awarded a grade of just D in this area. Unfortunately, this ended up being its second best grade. Short-term care grades are typically employed to gauge a facility's performance with rehabilitation. In order to offer highly graded rehabilitation services, facilities generally must offer better levels of skilled nursing services. Skilled nursing includes registered nurses, physical therapists, respiratory therapists and other licensed professionals. As you might expect, we discovered that this facility provides far less physical therapist and registered nurse hours per resident than a typical facility. Lastly, we assessed the number of patients who ultimately returned home from this facility. This facility didn't fare well here either. In fact, we found that just 4.1 percent of this facility's residents were able to return home.

Nurse Quality
We gave this facility a grade of just D for our nursing rating. Our nursing score is largely associated with a facility's level of nurse staffing. This particular nursing home averaged a meager 0.3 hours of nursing care per patient on a daily basis. This statistic is significantly below average. Finally, we also assessed some nursing quality-based metrics in computing our nursing grades. This facility performed well in the area of avoiding major falls and pressure ulcers. Many consider these metrics to be measures of the quality of nursing care.
Long-term Care Quality
The last area we scored is long-term care. We gave it a lowly F in this area, which is a rock bottom grade. For patients in need of a permanent residence as opposed to skilled nursing, long-term care grades are an important measure. One of the data points we considered after nurse's aid hours was vaccines. Thankfully, this nursing home administered the pneumonia vaccine to 96.58119 percent of its patients. Surprisingly, this facility also fared well at keeping its residents out of the hospital. Indeed, it had just 0.03 hospitalizations per 1,000 long-term resident days. Sadly, a few of its other scores in this category weren't as favorable as these.
