Facility Inspections
This facility earned a solid overall grade as described above, but we really wanted to emphasize its nearly flawless inspections in recent years. We gave them an A+ in this category. Inspection grades are tied to several items located in the a nursing home's recent inspection reports. Nursing homes that receive favorable grades in this area have few deficiencies on those reports. Most importantly, these facilities generally do not have any severe deficiencies which are associated with risks to patient safety. Amazingly, this was one of the few nursing homes in the country that had no deficiencies whatsoever on its inspection report. This is very impressive. Finally, this facility had no substantiated complaints this year from residents, which is an excellent sign.
Long-term Care Quality
This nursing home also fared well in the area of long-term care. In fact, we awarded it a score of A- in that category, which happens to be one of our highest grades. Facilities that excel in long-term care typically are well-staffed and offer extensive hands on care to patients. Once we assessed the volume of care provided by aids and other staff, we next considered the facility's vaccination record. This facility vaccinated 100 percent of its residents for pneumonia, which is much higher than the average nursing home. Lastly, this facility was able to limit hospitalizations. It had only 0 hospitalizations per 1,000 long-term resident days, which is a remarkably low figure.

Nurse Quality
Another strength of this facility is its favorable grade in nursing. We gave them one of our better scores in that area, with a grade of D. In calculating a facility's nursing rating, we assess the quantity of hours nurses spend with residents as well as the level of skill of those nurses. This place averaged only 0.4 hours of nursing care per patient daily. Lastly, we also assessed some nursing quality-based metrics in determining our nursing grades. This place fared well in the area of minimizing its residents' pressure ulcers and major falls. We consider these areas to be indicators of the quality of nursing care.
Short-term Care Quality
Moving on to our next area, this facility really did not perform very well. With a bottom of the barrel grade of F in short-term care, this was this nursing home's weakest category. In our short-term care grade, we strive to craft a valuable barometer for rehabilitation services. In doing so, we look at a facility's levels of highly-skilled nursing services, including both registered nurses and physical therapy, as well as speech therapy. Unfortunately, we discovered that this facility offers less registered nurse and physical therapist hours per resident than most nursing homes. The final measure we assessed in this category is the number of patients who ultimately returned home from the nursing home. This nursing home struggled quite a bit in this area as well, with just 3.8 percent of its residents returning home. Unfortunately, this was quite a bit below average.
