Facility Inspections

Grade: C

Although we gave this facility a terrible overall grade, we awarded it a C for our inspections rating. In fact, the facility received very impressive inspection report this year. We take several aspects of a facility's inspection report into consideration in calculating our inspection ratings. One key factor is health deficiencies. It should be noted that the severity of deficiencies is arguably more meaningful than the number of deficiencies, as some can be relatively minor. Amazingly, this was one of the few nursing homes in the country that had no deficiencies whatsoever on its inspection report. Unfortunately, we also need to draw your attention to the fact that this facility received significant government fines in recent years.

Nurse Quality

Grade: D

Turning to the area of nursing care, this facility received a grade of just D in this category. Unfortunately, this ended up being one of its better grades. Our nursing rating is primarily associated with the facility's nurse staffing. This nursing home provides 0.4 hours of nursing care per resident per day. This is a much lower figure than we are used to seeing. Finally, we also assessed some nursing quality measures in determining our nursing scores. This facility fared well in terms of minimizing its residents' pressure ulcers and major falls.

Long-term Care Quality

Grade: D

This facility's second least favorable score is long-term care, where it earned just a D in that area. This is a relatively poor grade. If you are seeking anything other than short-term rehabilitation, you should take a close look at each facility's long-term care ratings. One of the datapoints we considered in addition to nurse's aid hours is vaccines. Thankfully, this facility provided the pneumonia vaccination to 100 percent of its residents. To our surprise, this nursing home was able to limit hospitalizations. With only 0.02 hospitalizations per 1,000 long-term resident days, this nursing home had fewer hospitalizations than many nursing homes. Unfortunately, some of its other scores in the category were not as favorable as these.

Short-term Care Quality

Grade: F

Switching gears to the fourth area, this facility didn't fare very well in this category either. With a poor grade of F in short-term care, this is just about as as uninspiring as it gets. Our short-term care grades are arguably most critical for folks in need of rehabilitation services. Rehabilitation usually mandates additional highly-skilled nursing. Skilled nursing includes not merely nursing services, but also physical and respiratory therapy, as well as other forms of therapy. Not surprisingly, we found that this nursing home provides far less physical therapist and registered nurse hours per resident than the average facility. The final item we considered in this area is the percentage of residents who were able to leave the facility and return to the community. We discovered that just 4.4 percent of this facility's patients were able to return home, which is well below average. The combination of these subpar metrics sunk this nursing home's short-term care score.
