Facility Inspections
While we graded this facility incredibly poorly overall, it received a fairly respectable inspection report this year. As a result, it received a decent grade in that category with a score of B-. We weigh several aspects of a nursing home's inspection report in determining these inspection grades. One critical factor is deficiencies. You should keep in mind that the severity of deficiencies is arguably more important than the number of deficiencies, as some deficiencies end up being quite minor. Fortunately, although this facility had some minor dings on its inspection report, it had no severe deficiencies. Severe deficiencies are ones found in categories G, H, I, J, K and L. This means CMS did not deem any of the deficiencies on this facility's report to pose an imminent threat to patient safety or health. A few minor deficiencies aren't the end of the world.
Nurse Quality
One of the other reasons we graded this facility so highly is that it earned a very impressive nursing rating. In fact, nursing turned out to be its second best category grade. In that area, we gave this facility an D. There are a number of criteria within this category. Many of the criteria reflect levels of nurse staffing. This place provided a meager 0.4 hours of nursing care per resident each day. Lastly, we also looked at some nursing quality-based metrics in determining our nursing scores. This facility fared well in the area of avoiding major falls and pressure ulcers.

Short-term Care Quality
Unfortunately, this facility only earned a grade of D for its short-term care score, which is not a score to write home about. Our short-term care grade is generally used to grade a facility's performance with rehabilitation To offer good rehabilitation services, nursing homes generally need to offer better levels of skilled nursing. These types of services include registered nurses, physical therapists, respiratory therapists and other highly skilled individuals. Not surprisingly, we found that this nursing home provides far less physical therapist and registered nurse hours per resident than the average facility. The final item we looked at in this area is the number of patients who returned home from the nursing home. This facility struggled quite a bit in this metric as well, with just 4.5 percent of its residents returning home. This was quite a bit below the national average.
Long-term Care Quality
The next category we scored is long-term care, where this nursing home received a bottom of the barrel score here as well. Unfortunately, we gave it a lowly F for this area, which is obviously somewhat alarming. For long-term care residents, the primary goal is to maintain patients' quality of life and keep them safe. In addition to assessing the volume of care provided by nurses and other staff, we looked at the percentage of residents vaccinated against pneumonia. This facility gave the vaccine to 99.72299 percent of its patients, which is more than the vast majority of nursing homes. Surprisingly, this facility was able to limit hospitalizations. With just 0.01 hospitalizations per one thousand long-term resident days, this nursing home had fewer hospitalizations than the average nursing home. Unfortunately, some of its other scores in this category were not as favorable as these.
