Facility Inspections
While we graded this facility incredibly poorly overall, it received a fairly respectable inspection report this year. As a result, it received a decent grade in that category with a score of B-. Inspection grades weigh several factors included in a facility's inspection reports. One of the most important criteria we rely on is the number and severity of deficiencies. Although this place had a few deficiencies on its report, none were major deficiencies based on CMS' scale. Lastly, this facility was flagged by CMS for possible abuse or neglect. We prefer to avoid facilities with this designation. Please do your best to find an alternative nursing home.
Long-term Care Quality
Moving on to the category of long-term care, this facility received a grade of just D in that area. As it turns out, this proved to be one of its better features. Facilities that receive this kind of grade in long-term care likely don't provide the type of consistent 24/7 care that some other facilities offer. In addition to considering the volume of care provided by nurses and other staff, we looked at the percentage of residents vaccinated against pneumonia. This nursing home provided the vaccine to 93.5 percent of its residents. This is slightly below what we expected but still a reasonably acceptable percentage. To our surprise, this place was actually decent at keeping its patients out of the hospital. Indeed, it had just 0.04 hospitalizations per one thousand long-term resident days. This was its best score in this category.

Nurse Quality
We also wanted to draw your attention to this facility's poor nursing grade where it received an F. There are numerous subcategories within this category. Many of the factors relate to staffing levels. This facility provided only 0.3 hours of nursing care per resident on a daily basis. Lastly, we also looked at some nursing quality-based metrics in computing our nursing scores. This nursing home performed well in terms of avoiding pressure ulcers and major falls.
Short-term Care Quality
The next category we analyzed is short-term care. This facility was given an abysmal grade of F in that category. In our short-term care rating, we attempt to create a valuable gauge for rehabilitation services. In doing so, we look at a facility's scope of highly-skilled nursing services, including both registered nurses and physical therapy, as well as speech therapy. With its score in this area, we were not surprised to discover that this place was well below average in terms of the number of physical therapy and registered nurse hours offered to its patients based on the measures we assessed. Lastly, we assessed the percentage of residents who who were able to eventually return home from this facility. This nursing home didn't fare well here either. In fact, we found that just 4.6 percent of this facility's patients were able to return home. This figure was well off the national average.
