Facility Inspections
While we graded this facility incredibly poorly overall, it received a fairly respectable inspection report this year. As a result, it received a decent grade in that category with a score of B-. We believe that one of the most critical factors in assessing an inspection report is deficiencies. In this regard, we especially look at the severity of the deficiencies. Amazingly, this was one of the few nursing homes in the country that had no deficiencies whatsoever on its inspection report. This is very impressive. Lastly, this nursing home was cited by CMS for possible abuse or neglect. This is generally a really bad sign. We would never recommend this facility.
Long-term Care Quality
Incredibly, we awarded this facility a B- for our long-term care score, which is not a bad grade. Long-term care grades are generally used to assess a nursing home's performance as a traditional convalescent home, as opposed to focusing more on the skilled nursing services offered by a rehabilitation facility. Once we looked at the volume of care provided by aids and other staff, we then looked at the facility's vaccination record. We were disappointed to learn that this facility vaccinated a relatively low 51.383396 percent of its patients for pneumonia. Fortunately, this place was able to limit hospitalizations. It had less than one hospitalization per 1,000 long-term resident days, which is an impressively low figure.

Nurse Quality
Unfortunately, this facility only received a grade of D for its nursing grade, which was actually one of its better category grades. Nursing scores are tied to levels of nurse staffing. This facility offered just 0.5 hours of nursing care per patient on a daily basis. Lastly, we also assessed some nursing quality measures in computing our nursing scores. This nursing home fared well when it comes to avoiding pressure ulcers and major falls. Many consider these areas to be reliable indicators of the quality of nursing care.
Short-term Care Quality
The last category we looked at is short-term care, which turned out to be this nursing home's least impressive category. We awarded this facility a grade of D in that area. Our short-term care grade is generally employed to grade a facility's rehabilitation services. To offer quality rehabilitation services, facilities generally must provide higher levels of skilled nursing. Skilled nursing includes registered nurses, physical therapists, speech therapists and other licensed professionals. Not surprisingly, we found that this nursing home provides far less physical therapist and registered nurse hours per resident than the average facility. The last metric we considered in this category is the number of patients who were able to leave the facility and return to the community. We discovered that just 5 percent of this facility's residents were able to return home, which is well below average. The combination of these subpar metrics doomed this facility's short-term care grade.
