Facility Inspections
B-
Historical
Although we gave this facility a terrible overall grade, we awarded it a B- for our inspections rating. In fact, the facility received very impressive inspection report this year. We weigh several aspects of a nursing home's inspection report in determining these inspection grades. One critical factor is health deficiencies. You should keep in mind that the severity of the deficiencies is arguably more important than the quantity of deficiencies, as some are relatively insignificant. This particular nursing home received 5 deficiencies on its inspection report. On top of having severe deficiencies, this facility was cited by CMS for possible abuse or neglect. We would not recommend this facility under any circumstance.
Short-term Care Quality
D
Historical
Turning to the category of short-term care, this facility received a grade of just D in that category. As it turns out, this wound up being one of its better features. In calculating these short-term care ratings, we look at the nursing home's skilled nursing services, such as those performed by registered nurses, occupational therapists, physical therapists and other types of therapists. The goal is to devise a tool for sizing up the rehabilitation services of different nursing homes. Given its grade in this category, we weren't surprised to find that this place was well below average in terms of its quantity of physical therapy and registered nurse hours provided to its patients based on the measures we looked at. Finally, we considered the number of patients who were able to return to the community from this nursing home. We were surprised to find that it performed better in this area, with 50.6 percent of its residents returning home. This was just about this nursing home's sole highlight in this category.

Nurse Quality
D
Historical
We gave this facility a grade of just D for our nursing rating. Our nursing rating assesses the nursing home's level of nurse staffing. We weigh both the levels of training of the nurses and the number of hours spent with residents. This place offered only 3.2 hours of nursing care per patient daily. This is an alarmingly low total. Lastly, we also looked at a few quality measures in calculating our nursing scores. This place was above average in two of the datapoints we considered, with solid marks for minimizing its residents' pressure ulcers and major falls. These areas are often reliable indicators of the quality of nursing care offered.
Long-term Care Quality
F
Historical
Switching gears to the last area, this nursing home didn't perform very well in this area either. Due to a bottom of the barrel grade of F in long-term care, this is really as terrible as it gets. Facilities that do not score well in long-term care often don't provide as much nursing care and also may be struggling in a few of the areas of routine personal care we assessed. On top of looking at the amount of care provided by nurses and other staff, we also looked at the percentage of residents vaccinated against pneumonia. This facility administered the vaccine to 76.77419 percent of its patients. This is significantly lower than what we were hoping for. Surprisingly, this facility was actually able to limit hospitalizations. With just 0 hospitalizations per one thousand long-term resident days, this facility had fewer hospitalizations than many nursing homes. This was its best feature in this category.
