Facility Inspections
This nursing home earned a solid overall grade as described above, but we really wanted to emphasize its excellent government inspections in recent years. We gave them an A+ in this category. Inspection ratings are based on several items found on the a nursing home's recent inspection reports. Places that excel in this area have very few deficiencies on those reports. Most importantly, these facilities should not have any severe deficiencies which are associated with endangerment of patients. This facility was assessed 5 deficiencies on its inspection report, but fortunately none of the deficiencies were considered to be major deficiencies. This means that the inspectors did not deem any of these deficiencies to pose an immediate risk to patient safety or health. Finally, this facility had no substantiated complaints this year from residents, which is always a good sign.
Long-term Care Quality
Another one of this nursing home's better category scores came in the category of long-term care. In that area, we gave this nursing home a grade of B+. It actually fared better than most nursing homes in this area. Nursing homes that excel in long-term care tend to provide residents with better supervision and stay on top of routine healthcare services. In addition to looking at the quantity of care provided by nurses and other staff, we also looked at the percentage of residents vaccinated against pneumonia. This facility vaccinated 100 percent of its patients. Vaccines are critical to keeping residents out of the hospital. Clearly, this nursing home is doing something right in this area as it also excelled at keeping its patients out of the hospital. In fact, it had only 0.02 hospitalizations per 1,000 long-term resident days, which is a remarkably low number.

Short-term Care Quality
The next category we looked at was short-term care, where this nursing home received an F. Short-term care grades are often employed to assess a nursing home's rehabilitation services To provide highly graded rehabilitation services, facilities generally must offer better levels of highly skilled nursing services. These types of services include registered nurses, physical therapists, respiratory therapists and other highly skilled individuals. Considering its score in this category, we were not stunned to discover that this place is well below average in terms of the quantity of physical therapy and registered nurse hours provided to its residents based on the figures we assessed. Lastly, we looked at the number of patients who ultimately returned home from this facility. This facility didn't fare well here either. In fact, we found that just 5.2 percent of this facility's patients were able to return home. This figure was well off the national average.
Nurse Quality
The final area we graded was nursing. It received a lowly F for this area, which is an abysmal score. This is obviously a major concern. There are quite a few subcategories within this area. Most of these criteria relate to the quantity and quality of nurse staffing. This facility provided only 0.1 hours of nursing care per patient each day. Finally, we also looked at some nursing quality-based metrics in calculating our nursing grades. This nursing home fared well in the area of avoiding major falls and pressure ulcers.
